黄连温胆汤加风药治疗湿热型胃脘痛临床研究

摘　　要：目的:探讨黄连温胆汤加风药治疗脾胃湿热型胃脘痛患者的临床疗效。方法:选取在我院接受诊治的80例脾胃湿热型胃脘痛患者作为研究对象,按照随机数字表法分成实验组和对照组各40例。对照组采用黄连温胆汤治疗,而实验组则在对照组基础上依据患者临床表现辨证选用荆芥、防风、白芷、柴胡、升麻、葛根等风药,比较两组患者治疗前后的胃溃疡面积、胃黏膜积分、中医症状各项评分以及临床疗效。结果:实验组治疗后的胃黏膜溃疡面积、积分以及中医症状各项积分均明显低于对照组,差异具有统计学意义(P<0.05);实验组总有效率为95%(38/40),而对照组总有效率为80%(32/40),实验组总有效率明显高于对照组,差异具有统计学意义(P<0.05)。结论:黄连温胆汤加风药能有效改善脾胃湿热型患者的胃脘痛症状,同时辨证选用风药所带来的临床疗效也明显优于单纯使用黄连温胆汤治疗。Objective:To explore the clinical study of Huanglian Wendan Decoction plus Fengyao in the treatment of patients with epigastric pain of damp-heat type of spleen and stomach.Methods:Eighty patients with spleen and stomach moist heat type epigastric pain who were treated in our hospital from May 2017 to November 2019 were selected as the research subjects.They were divided into experimental group and control group according to the random number table method,with 40 cases in each group.The control group was treated with Huanglian Wendan Decoction,and the experimental group was based on the control group and selected Fengyao such as Jingjie,Fangfeng,Baizhi,Chaihu,Shengma,Gegen,etc,based on the clinical manifestations of patients.Compare the gastric ulcer area,gastric mucosal score,TCM symptom scores and clinical efficacy of the two groups of patients before and after treatment.Results:The gastric ulcer area,gastric mucosal score,TCM symptom scores after treatment in the experimental group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05);the total effective rate in the experimental group was 95%(38/40);the total effective rate of the control group was 80%(32/40).The total effective rate of the experimental group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Huanglian Wendan Decoction plus Fengyao can effectively improve the symptoms of epigastric pain in patients with spleen and stomach dampness and heat.At the same time,the clinical efficacy of dialectical selection of Fengyao is obviously better than Huanglian Wendan Decoction alone.
