从肾主纳气论益气温阳法治疗慢性阻塞性肺疾病合并肺动脉高压

摘　　要：以肾主纳气为理论指导,对慢性阻塞性肺疾病合并肺动脉高压中的缺氧机制进行分析和归纳,认为缺氧和肾不纳气是该病进程中两种相互耦联的病理状态。按病变累及的病位,可分三期论治,单纯COPD期肺气虚耗,气失所主,治疗当以补肺益气;COPD合并肺动脉高压期母病及子,肺肾两虚,治疗当以温肾助阳;右心衰竭期相火失位,君火不明,治疗当以振奋心阳。总之,纵观慢性阻塞性肺疾病合并肺动脉高压的病程,益气温阳法是治疗的关键,加味补阳还五汤作为益气温阳的代表方,对该病具有治疗作用。Under the guidance of the theory of kidney dominating Qi,the mechanism of hypoxia in COPD related pulmonary hypertension was analyzed and summarized.It was considered that hypoxia and kidney non Qi were two coupled pathological states in the process of COPD.According to the disease location involved in the disease,it can be divided into three stages:the first stage,the middle stage and the second stage.In the early stage,phlegm and blood stasis are blocked,and the lung qi is not good.The treatment should be to supplement the lung and Qi;in the middle stage,the mother’s disease,the son’s disease and the deficiency of the lung and kidney should be treated to warm the kidney and help Yang;in the later stage,when the phase of fire is lost,the monarch’s fire is unknown,the treatment should be to invigorate.In a word,throughout the course of chronic obstructive pulmonary disease-related pulmonary hypertension,the method of supplementing Qi and warming Yang is the key to treatment,and Buyang Huanwu Decoction,as the representative formula of supplementing Qi and warming Yang,has therapeutic effect on the disease.
