双钢板与Y型钢板治疗肱骨髁间骨折疗效的Meta分析

摘　　要：目的比较双钢板与Y型钢板治疗肱骨髁间骨折的临床疗效。方法运用计算机检索中国知网、维普、万方、Pubmed、Embase和Cochrane Library数据库,收集自建库至2019年11月20日国内外公开发表的有关双钢板与Y型钢板治疗肱骨髁间骨折疗效对比的随机对照研究,使用RevMan 5.3软件进行统计分析。结果本研究共纳入13篇文献,共计911名肱骨髁间骨折患者。其中双钢板组461例,Y型钢板组450例。Meta分析结果表明,两组手术方式在手术时间、术中出血量、住院时间方面均无统计学意义。而相较于Y型钢板组,双钢板组在骨折愈合时间上较短(MD=-11.06,95%CI:-16.24^-5.87,P<0.0001),术后并发症较少(MD=0.34,95%CI:0.21~0.55,P<0.0001),肘关节功能评分较高(MD=1.34,95%CI:1.23~1.47,P<0.0001)。结论双钢板在治疗肱骨髁间骨折时的骨折愈合时间、减少并发症及肘关节功能评分方面优于Y型钢板的固定方式。Objective To compare the efficacy of double-plate and Y-plate in the treatment of humeral intercondylar fracture.Methods A computer search was conducted on the databases of CNKI,WEIPU,WANFANG,Pubmed,Embase and Cochrane Library,and a randomized controlled study was collected from the self-built database to November 2019,which compared the efficacy of double-plate and y-plate in the treatment of humeral intercondylar fracture.RevMan 5.3 software was used for statistical analysis.Results A total of 13 literatures were included in this study,including 911 patients with humeral intercondylar fractures.There were 461 cases in the double-plate group and 450 cases in the y-plate group.The results of meta-analysis showed that the two groups had no statistical significance in terms of operation time,intraoperative blood loss and length of hospital stay.Compared with the y-plate group,the double-plate group had shorter fracture healing time(MD=-11.06,95%CI:-16.24^-5.87,P<0.0001),fewer postoperative complications(MD=0.34,95%CI:0.21~0.55,P<0.0001),and higher elbow joint function score(MD=1.34,95%CI:1.23~1.47,P<0.0001).Conclusion Double plate is better than Y plate in the treatment of intercondylar fracture of humerus in terms of fracture healing time,complication reduction and elbow joint function score.
