温肾排石汤联合经皮肾镜取石术对复杂性肾结石患者肾功能、神经体液因子及上皮细胞因子表达的影响  

Effects of Wenshen Paishi Decoction Combined with Percutaneous Nephrolithotomy on the Expression of Renal Function,Neurohumoral Factors and Epithelial Cytokines in Patients with Complex Renal Calculi

摘　　要：目的研究温肾排石汤联合经皮肾镜取石术(PCNL)治疗复杂性肾结石的疗效。方法选取60例复杂性肾结石患者,随机分为观察组和对照组,每组30例。对照组采用PCNL手术治疗。观察组在对照组治疗基础上给予温肾排石汤。比较两组的结石清除情况,检测两组治疗前后肾功能(CysC、Urea、Cr)、神经体液因子(hs-CRP、PCT、AngⅡ)及尿液炎症上皮细胞因子(MCP-1、TFF1、HMGB1)的表达水平。结果治疗后观察组清石率为97.67%(29/30),对照组清石率为86.67%(26/30),观察组清石率优于对照组(P<0.05)。治疗后两组Cr、Cys C、Urea水平指标水平均显著降低(P<0.05),且观察组Cr、Cys C、Urea水平指标水平均明显低于对照组(P<0.05)。治疗后两组hs-CRP、AngⅡ、PCT指标水平均明显降低(P<0.05),且观察组水平显著低于对照组(P<0.05)。治疗后两组MCP-1、HMGB1水平均明显降低,TFF1水平升高(P<0.05,P<0.01),且观察组较对照组MCP-1、HMGB1水平均明显降低,TFF1水平升高(P<0.05)。结论温肾排石汤在联合PCNL术治疗复杂性肾结石方面疗效突出,能改善肾功能,可有效降低神经体液因子及上皮细胞MCP-1、HMGB1的表达水平,提高TFF1表达水平,临床可以推广应用。Objective To study the efficacy of WenshenPaishiDecoction combined with percutaneous nephrolithotomy(PCNL)in the treatment of complex renal calculi.Methods 60 patients with complex renal calculi were randomly divided into observation group(30)and control group(30).The control group was treated with PCNL operation.The observation group was given WenshenPaishiDecoction based on the treatment of control group.The stone clearance of the two groups was compared,and the expression levels of renal function(CysC,Urea,Cr),neurohumoral factors(hs-CRP,PCT,AngⅡ)and urine inflammatory epithelial cytokines(MCP-1,TFF1,HMGB1)were detected before and after treatment.Results After treatment,the clearing rate of the observation group was 97.67%(29/30),the clearing rate of the control group was 86.67%(26/30),and the clearing rate of the observation group was better than that of the control group(P<0.05).After treatment,the levels of Cr,Cys C,Urea levels in the two groups were significantly reduced(P<0.05),and the levels of Cr,Cys C,Urea levels in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the levels of hs-CRP,AngⅡ,and PCT were significantly reduced in the two groups(P<0.05),and the observation group was significantly lower than the control group(P<0.05).After treatment,the levels of MCP-1 and HMGB1 in both groups were significantly reduced,and the level of TFF1 was increased(P<0.05,P<0.01),and the levels of MCP-1 and HMGB1 in the observation group were significantly lower than that of the control group,and the level of TFF1 was increased.(P<0.05).Conclusion WenshenPaishiDecoction is effective in the treatment of complex renal calculi combined with PCNL,which can improve renal function,effectively reduce the expression level of MCP-1 and HMGB1 in neurohumoral factors and epithelial cells,and increase the expression level of TFF1.It is worthy of application.
