益气养阴凉血解毒方治疗面部糖皮质激素依赖性皮炎61例临床观察

摘　　要：目的观察外用0.1%他克莫司软膏联合益气养阴凉血解毒方治疗面部糖皮质激素依赖性皮炎的临床疗效。方法选取2018年1月—2019年7月期间的面部糖皮质激素依赖性皮炎门诊患者121例为研究对象,采用随机数字表法随机分为治疗组61例和对照组60例。对照组患者给予0.1%他克莫司软膏皮损处适量外用,2次/d;治疗组患者早晚口服益气养阴凉血解毒方,2次/d,皮损部位应用益气养阴凉血解毒方水煎剂外洗后予0.1%他克莫司软膏外用治疗。两组均以4周为1个疗程。治疗后观察两组患者的皮肤瘙痒、灼热疼痛、肿胀潮红、干燥脱屑等临床症状的改善情况、临床治疗有效率、治疗前后用皮肤多功能测试仪检测患者的经皮水分丢失情况及皮肤红斑情况。结果治疗后治疗组有效率为86.89%(53/61),对照组有效率为74.14%(43/58),治疗组明显优于对照组(P=0.002<0.01)。治疗后,对照组患者皮肤红斑面积较治疗前减小(P<0.05),治疗组患者皮肤红斑面积较治疗前出现明显减小(P<0.05);两组间比较,治疗组患者皮肤红斑面积明显小于对照组(P<0.05)。对照组患者经皮水分丢失量较治疗前降低(P<0.05),治疗组较治疗前明显降低(P<0.05);两组间比较,治疗组经皮水分丢失量低于对照组患者(P<0.05)。治疗后两组患者皮肤瘙痒、灼热疼痛、肿胀潮红及干燥脱屑等症状积分变化较治疗前均出现不同程度改善(P<0.05),且治疗组改善较对照组明显(P<0.05)。结论他克莫司软膏联合益气养阴凉血解毒方能缩小面部糖皮质激素依赖性皮炎患者的皮肤红斑面积,减少皮肤水分丢失,改善患者皮肤瘙痒、灼热疼痛、肿胀潮红及干燥脱屑等临床症状,具有较高的临床有效率。Objective To observe the clinical efficacy of 0.1%tacrolimus ointment combined with YiqiYangyinLiangxueJiedu formula in the treatment of facial glucocorticoid dependent dermatitis.Methods 121 outpatients with facial glucocorticoid dependent dermatitis from January 2018 to July 2019 were randomly divided into treatment group and control group.There were 61 cases in the treatment group and 60 cases in the control group.The treatment group was treated with YiqiYangyinLiangxueJiedu formula twice a day,and the skin lesions were washed with YiqiYangyinLiangxueJiedu formula,and then 0.1%tacrolimus ointment was applied externally.The control group was treated only with 0.1%tacrolimus ointment externallytwice a day;one course for 4 weeks.After treatment,observe the improvement of clinical symptoms such as itching,burning pain,swelling and flushing,dryness and desquamation of the two groups of patients,the effectiveness of clinical treatment,and the skin multi-functional tester to detect the patients′percutaneous water loss and skin erythema before and after treatment.Results After one course of treatment,the effective rate of the treatment group and the control group were 86.9%and 74.1%respectively,and the treatment group was significantly better than the control group(P<0.01).After treatment,the area of skin erythema in the control group was reduced compared with that before treatment(P<0.05),and the area of skin erythema in the treatment group was significantly lower than that before treatment(P<0.05);compared between the two groups,the area of skin erythema in the treatment group was obvious Less than the control group(P<0.05).The transcutaneous water loss of the control group was lower than before treatment(P<0.05),and the treatment group was significantly lower than before treatment(P<0.05);compared between the two groups,the transdermal water loss of the treatment group was lower than that of the control group(P<0.05).After treatment,the scores of symptoms such as skin itching,burning pain,swelling,flushing,drynes
