渗湿消肿方外敷对复发性髌骨脱位术后关节运动障碍及康复质量的影响

摘　　要：目的探讨渗湿消肿方外敷对复发性髌骨脱位术后关节运动障碍及康复质量的影响。方法选取2018年1月—2019年1月收治的96例复发性髌骨脱位需行手术治疗患者,随机分为对照组48例、治疗组48例。对照组患者给予口服塞来昔布胶囊治疗,治疗组患者在对照组基础上联合使用渗湿消肿方外敷。比较两组患者关节影像学指标、关节功能评分、康复质量、不良反应。结果治疗后治疗组髌骨-股骨滑车适合角、Q角小于对照组,差异有统计学意义(P<0.05),髌骨外侧角高于对照组,差异有统计学意义(P<0.05);治疗后治疗组Lysholm评分高于对照组,差异有统计学意义(P<0.05);治疗后治疗组VAS评分低于对照组,差异有统计学意义(P<0.05);治疗组患者随访过程中患侧膝关节不良症状发生率4.17%显著低于对照组18.75%,差异有统计学意义(P<0.05);对照组不良反应发生率6.28%略高于治疗组4.17%,但比较差异无统计学意义(P>0.05)。结论对复发性髌骨脱位患者术后采用渗湿消肿方外敷,能显著改善患者膝关节功能,缓解关节疼痛,提高康复质量,且安全性较高。Objective To investigate the effect of external application of Shenshi Xiaozhong formula on joint dyskinesia and rehabilitation quality of recurrent patella dislocation patients after surgery.Methods A total of 96 patients with recurrent patella dislocation underwent surgery admitted to the hospital from January,2018 to January,2019 were enrolled,and randomly divided into a control group and an observation group,with 48 cases in each group.The control group received the celecoxib capsules,and the observation group received the external application of Shenshi Xiaozhong Formula on the basis of the control group.The imaging parameters,joint function score,rehabilitation quality,and adverse reactions were compared between the two groups.Results After treatment,the patella-femur congruence angle and Q-angle in the observation group were smaller than those in the control group,with statistical significance(P<0.05),and the lateral patella angle was higher than that in the control group,with statistical significance(P<0.05).The observation group had higher Lysholm score and lower VAS score than those of the control group after treatment(P<0.05).During follow-up,the adverse reactions of knee joint at the affected side in the observation group(4.17%)was significantly lower than that in the control group(18.75%),with statistical significance(P<0.05).The rate of adverse events in the control group(6.28%)was slightly higher than in the observation group(4.17%),without statistical significance(P>0.05).Conclusion External application of Shenshi Xiaozhong Formula can effectively and safely improve the knee joint function,relieve joint pain,and improve the quality of rehabilitation for patients with recurrent patella dislocation.
