低分子右旋糖酐氨基酸注射液致心动过速1例

摘　　要：1例70岁男性患者,术后给予低分子右旋糖酐氨基酸注射液补充血容量.当患者输注低分子右旋糖酐氨基酸注射液剩余约100mL时,出现胸腹部斑丘疹伴瘙痒,心率达到220次/min,血压降至58/35mm Hg.考虑为低分子右旋糖酐氨基酸注射液所致过敏反应,立即给予0.9%氯化钠静脉滴注、地塞米松磷酸钠注射液静脉推注、多巴胺注射液静脉滴注、去乙酰毛花苷注射液静脉推注、盐酸异丙嗪肌肉注射等抗过敏治疗.2h后患者血压升至135/80mm Hg,心率下降到120次/min,氧饱和度100%,全身散在斑丘疹消退,不适症状基本缓解.A 70-year-old male patient was given low molecular dextran amino acid injection to supplement blood volume after an operation.When about 100 mL of low molecular dextran amino acid injection remained,the patient developed maculopapulae with pruritus in the chest and abdomen,his heart rate(HR) reached 220 beats/minute,and his blood pressure(BP) dropped to 58/35 mm Hg.With allergic reaction caused by low molecular dextran amino acid injection being taken into consideration,antiallergic treatment covering 0.9% sodium chloride through an intravenous drip,dexamethasone sodium phosphate injection through intravenous injection,dopamine injection through an intravenous drip,deslanoside injection through intravenous injection,promethazine hydrochloride through intramuscular injection,etc. was given immediately.After 2 h,the patient’s BP increased to 135/80 mm Hg,HR decreased to 120 beats/minute,oxygen saturation was 100%,maculopapulaes scattered all over the body subsided,and discomfort symptoms were basically relieved.
