摘 要:目的探讨黄芪保心汤对异丙肾上腺素诱导慢性心力衰竭大鼠的保护作用及其可能作用机制。方法将60只雄性SD大鼠随机分为对照组、模型组、黄芪保心汤组、卡托普利组,每组15只。对照组不造模,其余组均采用异丙肾上腺素连续皮下注射7 d复制心力衰竭模型。建模成功后,黄芪保心汤组大鼠给予黄芪保心汤5 mL/kg灌胃,4次/d,连续4周;卡托普利组大鼠给予卡托普利50 mg/kg灌胃,对照组和模型组给予等体积的生理盐水灌胃,均1次/d,连续4周。心脏彩超检查大鼠心脏功能,记录左室射血分数(LVEF)、左室收缩期内径(LVDs)、左室舒张期内径(LVDd)、短轴缩短率(FS);HE染色和Masson染色观察大鼠心肌组织病理变化,DCFH-DA染色检测大鼠心肌细胞内活性氧(ROS)水平,Rh123染色观察大鼠心肌细胞线粒体膜电位变化,Western blot法检测大鼠心肌组织中凋亡相关蛋白Bax和Bcl-2的表达水平。结果与对照组比较,模型组大鼠LVDs、LVDd明显升高(P均<0.05),LVEF、FS明显降低(P均<0.05);与模型组比较,黄芪保心汤组、卡托普利组大鼠LVDs、LVDd明显降低(P均<0.05),LVEF、FS明显升高(P均<0.05)。模型组大鼠心肌细胞肥大,细胞外基质、成纤维细胞增多,见大量心肌胶原纤维;黄芪保心汤组、卡托普利组大鼠心肌细胞较模型组排列规则,纤维组织增生和胶原纤维明显减少。与对照组比较,模型组大鼠心肌细胞ROS荧光吸光度值明显增高(P<0.05),Rh123荧光吸光度值明显降低(P<0.05);与模型组比较,黄芪保心汤组、卡托普利组大鼠心肌细胞ROS荧光吸光度值明显降低(P均<0.05),Rh123荧光吸光度值明显升高(P均<0.05)。与对照组比较,模型组大鼠心肌组织中Bax蛋白表达水平明显升高(P<0.05),Bcl-2蛋白表达水平和Bcl-2/Bax明显降低(P均<0.05);与模型组比较,黄芪保心汤组、卡托普利组大鼠心肌组织中Bax蛋白表达水平明显降低(P均<0.05),Bcl-2蛋白表达水平�Objective It is to explore the protective effect of Huangqi Baoxin decoction on isoproterenol-induced chronic heart failure in rats and its possible mechanism.Methods Sixty male SD rats were randomly divided into control group,model group,Huangqi Baoxin decoction group,captopril group,15 rats in each group.The models were not established in the control group,and the heart failure models were established by continuous subcutaneous injection of isoproterenol for 7 days in the other groups.After successful modeling,the rats in the Huangqi Baoxin decoction group were treated with Huangqi Baoxin decoction 5 mL/kg by gavage,4 times per day for 4 weeks;the rats in the captopril group were given captopril 50 mg/kg by gavage,and the ones in the control group and model group were given an equal volume of normal saline by gavage,all once a day for 4 weeks.The cardiac function of the rats were detected by cardiac color Doppler ultrasound,left ventricular ejection fraction(LVEF),left ventricular end-systolic dimension(LVDs),left ventricular end-diastolic diameter(LVDd),fraction shortening(FS)were recorded;the pathological changes in rat myocardial tissue were observed by HE staining and Masson staining,the level of reactive oxygen species(ROS)in rat myocardial cells was detected by DCFH-DA staining,the changes of mitochondrial membrane potential of rat myocardial cells was observed by Rh123 staining,and the expression level of apoptosis-related protein Bax and Bcl-2 in rat myocardial tissue was detected by Western blot method.Results Compared with the control group,the LVDs and LVDd of the model group were significantly increased(P<0.05),and LVEF and FS were significantly decreased(P<0.05);compared with the model group,LVDs and LVDd of the Huangqi Baoxin decoction group and the captopril group were significantly reduced(all P<0.05),and LVEF and FS were significantly increased(all P<0.05).The cardiomyocytes of the model group were hypertrophy,extracellular matrix and fibroblasts increased,and a large number of myocardial colla
