摘　　要：目的探讨促性腺激素释放激素(GnRH)激动剂辅助腹腔镜手术治疗子宫内膜异位症的临床疗效。方法纳入2015年1月-2018年7月深圳市龙岗区妇幼保健院收治的90例子宫内膜异位症患者,按随机数表法分为对照组和观察组各45例。对照组行腹腔镜手术,观察组在对照组的基础上加用GnRH戈舍林肌注治疗。观察并比较治疗后总有效率,治疗前后血清性激素的变化、复发率及妊娠率。结果治疗后,观察组患者治疗总有效率为91.11%,明显高于对照组的80.00%,差异有统计学意义(P<0.05)。两组患者的血清FSH、LH和E2水平均明显低于治疗前,且观察组明显低于对照组,差异均有统计学意义(P<0.05)。观察组的复发率为4.44%,低于对照组的20.00%;观察组的妊娠率为66.67%,明显高于对照组的33.33%,差异均有统计学意义(P<0.05)。结论GnRH激动剂辅助腹腔镜手术治疗子宫内膜异位症能明显提高治疗总有效率,抑制性激素水平,降低患者复发率,值得临床推广应用。Objective To investigate the clinical effect of gonadotropin-releasing hormone(GnRH)agonist assisted laparoscopic surgery in the treatment of endometriosis.Methods Ninety patients with endometriosis who were admitted to Longgang District Maternal and Child Health Hospital of Shenzhen fro m January 2015 to July 2018 were enrolled and divided into control group and observation group with 45 cases in each group according to the random number table method.The control group underwent laparoscopic surgery,and the observation group was treated with GnRH agonist goserelin intramuscular injection on the basis of the control group.The total effective rate after treatment,the changes of serum hormones,recurrence rate and pregnancy rate before and after treatment were observed and compared.Results After treatment,the total effective rate in the observation group was 91.11%,significantly higher than 80.00%in the control group,and the difference was statistically significant(P<0.05).The serum FSH,LH and E2 levels in the two groups were significantly lower than those before treatment,and the observation group was significantly lower than the control group,and the differences were statistically significant(P<0.05).The recurrence rate of the observation group was 4.44%,lower than 20.00%of the control group;the pregnancy rate of the observation group was 66.67%,significantly higher than 33.33%of the control group,and the differences were statistically significant(P<0.05).Conclusion GnRH agonist assisted laparoscopic surgery for endometriosis could significantly improve the total effective rate,inhibit the level of sex hormones,reduce the recurrence rate of patients,which was worthy of clinical promotion and application.
