摘  要:目的:探讨有创呼吸机实施无创正压通气对比常规吸氧气治疗急性心源性肺水肿的临床价值。方法:选取2017年10月~2019年9月本院治疗的92例急性心源性肺水肿患者为研究对象并随机分为观察组和对照组各46例。所有患者均采取常规治疗,进行心电监护,对血压(BP)、血氧饱和度(SPO2)、呼吸(RR)、心率(HR)等指标进行监测,进行抗心衰治疗。观察组患者采取无创正压通气,使用BiPAPST—D30呼吸机,S/T工作模式,IPAP从6cmH2O开始,EPAP从3 cmH2O开始,呼吸频率10~18次/min。对比两组患者治疗后pH、SPO2、二氧化碳分压(PaCO2)、氧合指数(PaO2/FiO2)、HR等有无差异。结果:治疗后,观察组患者的pH、SPO2、PaO2/FiO2等指标均明显高于对照组,PaCO2、HR等指标明显低于对照组(P<0.05)。结论:对患者进行无创正压通气可取得更加显著的辅助治疗效果,明显优于常规吸氧,应用效果良好。Objective:To explore the clinical value of noninvasive positive pressure ventilation in invasive ventilator versus conventional oxygen inhalation in the treatment of acute cardiogenic pulmonary edema.Methods:92 patients with acute cardiogenic pulmonary edema treated in our hospital from October 2017 to September 2019 were selected as the research subjects and randomly divided into observation group and control group,46 cases each.All patients were treated with conventional treatment,ECG monitoring,blood pressure(BP),blood oxygen saturation(SPO2),respiration(RR),heart rate(HR)and other indicators were monitored,and anti-heart failure treatment was performed.Patients in the observation group used non-invasive positive pressure ventilation,using BiPAPST-D30 ventilator,S/T working mode,IPAP started from 6cmH2O,EPAP started from 3cmH2O,and the respiratory rate was 10-18 times/min.Compare the difference of pH value,SPO2,carbon dioxide partial pressure(PaCO2),oxygenation index(PaO2/FiO2)and HR after treatment between the two groups.Results:After treatment,the observation group’s pH,SPO2,PaO2/FiO2 and other indicators were significantly higher than the control group,PaCO2,HR and other indicators were significantly lower than the control group(P<0.05).Conclusion:Non-invasive positive pressure ventilation can achieve more significant adjuvant treatment effect,which is significantly better than conventional oxygen inhalation,and the application effect is good.
