阿替普酶静脉溶栓治疗急性ST段抬高型心肌梗死的临床观察

摘　　要：目的观察阿替普酶静脉溶栓治疗急性ST段抬高型心肌梗死的临床效果。方法选取2016年12月-2018年6月四川省彭州市人民医院心血管内科收治的急性ST段抬高型心肌梗死患者80例,按照入院单双序号随机分为观察组和对照组各40例。观察组患者采用阿替普酶静脉溶栓治疗,对照组采用实施尿激酶静脉溶栓治疗。比较2组患者治疗前后心肌酶水平、肌钙蛋白T水平、血管再通率及并发症发生情况。结果治疗后,观察组肌钙蛋白T、乳酸脱氢酶、超氧化物歧化酶及血清肌酸激酶同工酶水平均优于对照组(P<0.05)。观察组患者血管再通率为87.50%,高于对照组患者的75.00%(P<0.05)。观察组患者并发症总发生率为17.50%,低于对照组患者的40.00%(P<0.05)。结论阿替普酶静脉溶栓治疗急性ST段抬高型心肌梗死疗效显著,可提高冠脉再通率,降低不良反应发生率,安全可靠,值得在临床工作中应用。Objective To observe the clinical effect of alteplase intravenous thrombolysis on acute ST-segment elevation myocardial infarction.Methods 80 patients with acute ST-segment elevation myocardial infarction admitted to the Department of Cardiology,Pengzhou People’s Hospital,Sichuan Province from December 2016 to June 2018 were selected,and they were randomly divided into observation group and control group according to the number of admissions,40 cases in each group.The observation group was treated with alteplase intravenous thrombolysis,and the control group was treated with urokinase intravenous thrombolysis.The levels of myocardial enzyme,troponin T,vascular recanalization rate and complications before and after treatment were compared between the two groups.Results After treatment,the levels of troponin T,lactate dehydrogenase,superoxide dismutase and serum creatine kinase isoenzymes in the observation group were better than those in the control group(P<0.05).The recanalization rate of the observation group was(87.50%)higher than that of the control group(75.00%)(P<0.05).The incidence of complications in the observation group was(17.50%)lower than in the control group(40.00%)(P<0.05).Conclusion Intravenous thrombolysis with alteplase is effective in the treatment of acute ST-segment elevation myocardial infarction,it can increase the rate of coronary recanalization,reduce the incidence of adverse reactions,it is safe and reliable,and it is worthy of clinical application.
