医养结合—四元联动整合照护模型的SWOT-CLPV分析——以合肥市为例

摘　　要：目的旨在评价合肥市在实践医养结合—四元联动整合照护模型方面的抑制性、杠杆作用、问题性和脆弱性等因素,为进一步优化此模型在医联体单位合作中的应用效果提供参考。方法采用SWOT-CLPV理论对医养结合—四元联动整合照护模型的实施现状及存在的问题进行分析。结果合肥市率先采取医养结合—四元联动模型,通过医院、护理院、社区及家庭之间的密切联系,形成了医疗资源的多向流动,促进了医联体单位间的沟通与协作,能够为老年人提供更为便利的养老就医渠道。但是如何进一步深化此模型应用效率成为今后工作的重点难点。医养结合已成为我国在养老方面的重要战略部署,在人口老龄化不断深化的当下,践行医养结合—四元联动模型既是顺应国家政策的优势举措,又面临多方面挑战。结论合肥市在实践医养结合—四元联动整合照护模型中,完善了医联体单位之间的协作机制,就医养老服务的流动性较好。各级医疗卫生单位需要丰富四元联动模型的实施对策,把握当前的养老政策新态势,以满足合肥市本地区老年人的就医—养老需求为中心目标,推动医养结合—四元联动整合照护模型的高效运转。OBJECTIVE To explore the control,leverage,problem and vulnerability of the practice of the"treatment-care and four linkage combination"integrated care model in Hefei,and provide references for further optimizing the application effect of this model in the cooperation of medical association units.METHODS The SWOT-CLPV model was used to analyze the implementation status and remaining problems of the"treatment-care and four linkage combination"integrated care model.RESULTS Hefei has taken the lead in adopting the"treatment-care and four linkage combination"integrated care model.Through the close connection among hospitals,medical institutions,communities and families,it had formed a multi-directional flow of medical resources and promoted communication and collaboration among medical federation units.The practice of the model provided more convenient channels for the elderly to seek medical care.CONCLUSION In the practice of the"treatment-care and four linkage combination"integrated care model,Hefei improved the cooperation mechanism among medical consortia units,and the mobility of medical care and elderly care services was better.Medical and health units at all levels needed to enrich the implementation of the quaternary linkage model,grasp the current situation of the old-age care policy,meet the elderly-healing needs of the elderly in the region of Hefei and promote the efficient operation of the"treatment-care and four linkage combination"integrated care model.
