含多西环素四联疗法对伴幽门螺杆菌感染的丘疹脓疱型玫瑰痤疮疗效观察

摘　　要：目的 含多西环素四联疗法对伴幽门螺杆菌(Hp)感染的丘疹脓疱型玫瑰痤疮疗效和安全性.方法 门诊进行14C-尿素呼气试验(14C -UBT)检测的丘疹脓疱型玫瑰痤疮患者95例,其中阳性60例,阴性35例,将检测阳性病例随机分为试验组(30例)和对照组(30例),其中试验组采用根除Hp四联疗法(多西环素肠溶胶囊100mg+阿莫西林胶囊1000mg+奥美拉唑肠溶片20mg+枸橼酸铋钾胶囊300mg,均每日口服两次),疗程共2周;对照组口服多西环素100mg,每日一次,疗程共8周;两组均联合甲硝唑凝胶外用,每日2次,共8周.记录并对比治疗前和治疗2、8、12周的丘疹脓疱计数、丘疹脓疱的研究者整体评估(IGA)分级、Hp根除疗效、不良反应、玫瑰痤疮生活质量(RosaQoL)和服药依从性.结果 治疗2周试验组患者丘疹脓疱计数较对照组明显减少(P<0.05),其中试验组有效率达86.67%,对照组为63.33%,差异具有统计学意义(P=0.04<0.05);8周和12周后,两组有效率差异无统计学意义(P>0.05);Hp感染转阴率试验组为86.67%,对照组43.33%,差异有统计学意义(P=0.03<0.05).结论 含多西环素四联疗法与传统治疗具有相似疗效和不良反应,但Hp根除率更高,且四联疗法用药时间更短、见效更快、依从性更高,值得进一步研究和临床推广.Objective To evaluate the efficacy and safety of quadruple therapy containing doxycycline for the treatment of Papulopustular rosacea with H.pylori infection.Methods 95 cases of Papulopustular rosacea were detected by 14C-urea breath test(14C-UBT)in outpatient clinics,of which 60 were positive and 35 were negative.The positive cases were randomly divided into experimental groups(30 cases)and control group(30 cases),The experimental group was treated with quadruple therapy(doxycycline enteric-coated capsules100 mg+amoxicillin capsules 1000mg+omeprazole enteric-coated tablets 20mg+bismuth potassium citrate capsules 300mg,both taken orally twice a day)for a total of 2 weeks;The control group was treated with doxycycline,100mg once daily for 8 weeks.Both groups were combined with topical metronidazole gel,twice a day for 8 weeks.The primary endpoint was the absolute change in inflammatory lesion count and grade 2 or higher Investigator’s Global Assessment(IGA)improvement at weeks 0,2,8 and 12.Other assessments included Efficacy of Hp eradication therapy,treatment-related adverse events,Rosacea Quality of Life Index(RosaQoL)And medication compliance.Results The inflammatory Papules and pustules in the experimental group were significantly reduced compared with the control group(P<0.05)after 2 weeks of treatment.The effective rate of the experimental group was 86.67%,and the control group was 63.33%,The comparison between the two groups was statistically significant(P=0.04<0.05),but not statistically significant after 8 and 12 weeks of treatment(P>0.05).The eradication rate of H.pylori infection was 86.67%in the experimental group and 43.33%in the control group,and the difference was statistically significant(P=0.03<0.05).Conclusion Quadruple therapy has similar efficacy and adverse reactions as traditional therapy,but it has a higher H.pylori eradication rate.Quadruple therapy,which deserves further research and clinical promotion,has shorter medication time,faster response,and higher compliance than traditional the
