摘　　要：目的研究活血通络方联合针刺治疗腰椎间盘突出症的临床疗效及机制。方法纳入2016年7月至2018年7月我院收治的84例腰椎间盘突出症患者,随机均分为对照组和观察组,每组42例。对照组给予针刺、药物等常规治疗,观察组在此基础上联合活血通络方治疗,两组均连续治疗2个疗程(共20 d)。比较两组患者治疗前、后血清炎性因子、丙二醛(malondialdehyde,MDA)、超氧化物歧化酶(superoxide dismutase,SOD)水平、疼痛视觉模拟(visual simulation of pain,VAS)评分及中医症候指标,评价其综合疗效。结果观察组治疗总有效率(88.10%)高于对照组(76.19%),差异具有统计学意义(P<0.05)。治疗后,两组血清C反应蛋白、白介素-6及肿瘤坏死因子水平均较治疗前降低,且治疗后观察组上述指标均低于对照组,差异均具有统计学意义(P<0.05)。治疗后,两组中医症候指标均较治疗前降低,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组SOD水平均较治疗前上升,MDA水平及疼痛VAS评分均降低,且治疗后观察组SOD水平较对照组高,MDA水平及疼痛VAS评分较对照组低,差异均具有统计学意义(P<0.05)。结论活血通络方联合针刺治疗腰椎间盘突可有效改善疼痛症状,提高临床疗效,且可降低炎症反应及过氧化损伤。Objective To study the therapeutic effect and mechanism of Huoxuetongluo recipe combined with acupuncture on lumbar disc herniation.Methods Eighty-four patients with lumbar disc herniation admitted to our hospital from July 2016 to July 2018 were included,and randomly divided into the control group and the observation group.The control group was given conventional treatments such as acupuncture and drugs,and the observation group was treated with this combined Huoxuetongluo recipe on this basis,and continuous treatment for 2 courses(20 days).Serum inflammatory factors,malondialdehyde(MDA),superoxide dismutase(SOD)levels,pain visual analogue(VAS)scores and traditional Chinese medicine(TCM)syndrome index were compared between the two groups before and after treatment.The comprehensive efficacy were evaluated.Results The total effective rate of the observation group and the control group were 88.10%and 76.19%.The observation group was higher than the control group,and the difference was significant(P<0.05).The C-reactive protein,interleukin-6 and tumor necrosis factor levels of both groups before treatment were significantly lower those after treatment,and those of the observation group were lower than those of the control group.The difference was statistically significant(P<0.05).TCM syndrome index was lower after treatment in both groups,and the observation group was significantly lower than the control group.The difference was statistically significant(P<0.05).Serum SOD levels increased significantly after treatment,MDA levels and pain VAS scores were decreased,and the observation group was superior to the control group after treatment,the difference was statistically significant(P<0.05).Conclusion The Huoxuetongluo recipe combined acupuncture for the treatment of lumbar intervertebral discs can significantly reduce the symptoms of pain and improve the clinical efficacy.The acupuncture combined medicine treatment can reduce the inflammatory response and peroxidative damage.
