龙胆泻肝汤在皮肤病中的应用

摘　　要：皮肤病辨证多以肝胆湿热、肝胆实火为主。龙胆泻肝汤是治疗肝胆湿热证的代表方,本是清利肝胆湿热之剂,虽以“清泻”为主,但泻中有补,避免了过度清泻导致耗伤人体阴液的情况。此外,足厥阴肝经与足少阳胆经相互属络,互为表里,一升一降,二者同治,方可奏效。综观全方,龙胆泻肝汤配伍涉及阴阳、脏腑、经络、表里、寒热、气血、升降、补泻等,利湿兼顾滋阴,能使热清火泻、湿浊清利。龙胆泻肝汤常用于治疗感染性皮肤病、过敏性皮肤病、红斑鳞屑性皮肤病、神经精神障碍性皮肤病、皮肤附属器疾病等,临证时需根据患者症状、体征、舌脉等进行辨证加减,常获良效。Syndrome differentiation of skin diseases mainly includes dampness and heat and excess fire of liver and gall.Gentian Liver-Draining Decoction is a representative prescription for the treatment of damp heat syndrome of liver and gall.Although it is mainly used for"pure diarrhea",it can supplement the diarrhea,so as to avoid the excessive diarrhea leading to the consumption of yin fluid.In addition,liver meridian of foot jueyin and gallbladder meridian of foot shaoyang are connected with each other,and they have exterior-interior relationship;one ascends and the other descends.Only are they treated together,efficacy can be attained.In a comprehensive view,the compatibility of Gentian Liver-Draining Decoction involves yin and yang,zang-fu organs,meridians,exterior and interior,cold and heat,qi and blood,ascending and descending,tonifying and reducing,etc.It can eliminate dampness,nourish yin,clear heat and purge fire,remove turbidity and promote diuresis.Gentian Liver-Draining Decoction is often used to treat various skin diseases such as infectious dermatosis,allergic dermatosis,erythematous scaly dermatosis,neurodysfunctional dermatosis,diseases of skinappendages,etc.In clinical practice,the patients′symptoms,signs,tongue pulse,etc.should be considered for modification based on syndrome differentiation,and good effect is usually obtained.
