苦参解毒合剂坐浴联合切开挂线疗法治疗高位肛周脓肿临床研究

摘　　要：目的:观察苦参解毒合剂坐浴联合切开挂线疗法治疗高位肛周脓肿的临床疗效。方法:选择2017年5月至2019年5月在本院住院的高位肛周脓肿患者70例,随机分为对照组和观察组各35例。对照组采用肛周脓肿挂线引流术,术后使用高锰酸钾坐浴治疗。观察组采用肛周脓肿挂线引流术,术后使用苦参解毒合剂坐浴治疗。两组疗程均为28 d,疗程结束后对两组患者的临床疗效、住院时间、脓腔愈合时间、腐肉脱落时间及疼痛视觉模拟评分(visual analogue score,VAS)进行比较。结果:对照组有效率为82.6%,观察组有效率为97.1%,两组有效率比较,差异有统计学意义(P<0.05)。观察组住院时间、脓腔愈合时间、腐肉脱落时间与对照组比较均明显缩短,差异具有统计学意义(P<0.05)。治疗后7 d、14 d、21 d、28 d,观察组VAS评分均低于对照组,差异具有统计学意义(P<0.05)。随访1年后,观察组脓肿复发率、创面感染率、功能障碍发生率均明显低于对照组,差异具有统计学意义(P<0.05)。结论:苦参解毒合剂联合切开挂线疗法治疗高位肛周脓肿可以明显减轻患者疼痛,促进创面愈合,减少并发症。Objective:To observe the clinical curative effect of Kuh-Seng Toxin-Removing Mixture combined with incision and thread-drawing therapy on high perianal abscess.Methods:A total of 70 patients with high perianal abscess treated in our hospital from May 2017 to May 2019 were randomly divided into the control group and the observation group,with 35 cases in each group.The control group was treated with thread-drawing drainage of perianal abscess and potassium permanganate for hip bath after operation.The observation group was treated with thread-drawing drainage of perianal abscess and hip bath with Kuh-Seng Toxin-Removing Mixture after operation.The course of treatment lasted 28 days in both groups.After the course of treatment,the clinical efficacy,hospitalization time,healing time,carrion-shedding time and visual analogue score(VAS)were compared between the two groups.Results:The effective rate of the control group was 82.6%,and that of the observation group was 97.1%.The difference between the two groups was statistically significant(P<0.05).Compared with the control group,the hospitalization time,the healing time of pus cavity and the carrion-shedding time in the observation group were significantly shorter than those in the control group,and all the differences were statistically significant(P<0.05).On the 7 th,14 th,21 st and 28 th days after the treatment,all the VAS scores of the observation group were lower than those of the control group,and all the differences were statistically significant(P<0.05).After one year of follow-up,the abscess recurrence rate,wound infection rate and dysfunction rate in the observation group were significantly lower than those in the observation group,and all the differences were statistically significant(P<0.05).Conclusion:In terms of treating high perianal abscess,Kuh-Seng Toxin-Removing Mixture combined with incision and thread-drawing therapy can significantly reduce pain,promote wound healing and reduce the incidence of complications of the patients.
