整脊手法松解髂腰肌治疗退行性腰椎滑脱症疗效观察

摘　　要：目的:观察整脊手法松解髂腰肌治疗退行性腰椎滑脱症(degenerative lumbar spondylolishthesis,DLS)的临床疗效。方法:纳入研究的60例患者均为本院2015年1月至2019年12月收治的DLS患者,采用随机数字表法分为治疗组和对照组,每组30例。对照组给予常规手法治疗,治疗组在对照组治疗的基础上给予手法松解髂腰肌治疗。采用疼痛视觉模拟评分(visual analogy score,VAS)、日本骨科协会(Japanese ortho-paedic association,JOA)下腰痛评分法评估两组患者的疼痛情况。结果:治疗组治疗后VAS评分低于对照组,差异有统计学意义(P<0.05);治疗组治疗后JOA评分高于对照组,差异有统计学意义(P<0.05);治疗组和对照组优良率分别为93.33%和76.67%,治疗组和对照组愈显率分别为70.0%和50.0%,治疗组优良率和愈显率均高于对照组,差异有统计学意义(P<0.05)。结论:整脊手法松解髂腰肌治疗退行性腰椎滑脱症,可减轻患者疼痛,改善VAS评分、JOA评分。Objective:To observe the clinical curative effect of method of rotation and reduction of spine fixation(fixed point)for releasing iliopsoas muscle on degenerative lumbar spondylolishthesis(DLS).Methods:The 60 patients for study were all DLS patients admitted to our hospital from January 2015 to December 2019.They were randomly divided into the treatment group and the control group,with 30 cases in each group.The control group was given conventional manipulation therapy,while the treatment group was given method of rotation and reduction of spine fixation(fixed point)for releasing iliopsoas muscle on the basis of the control group.Visual analogue score(VAS)and Japanese Orthopaedic Association(JOA)low back pain score were used to evaluate the pain of the two groups.Results:After the treatment,the VAS score of the treatment group was lower than that of the control group,the difference was statistically significant(P<0.05);the JOA score of the treatment group was higher than that of the control group,the difference was statistically significant(P<0.05);the excellent rates of the treatment group and the control group were 93.33%and 76.67%respectively,and the remarkably effective rates of the treatment group and the control group were 70.0%and 50.0%respectively,and the excellent rate and the remarkably effective rate of the treatment group were all higher than those of the control group,and all the differences were statistically significant(P<0.05).Conclusion:In terms of treating DLS,method of rotation and reduction of spine fixation(fixed point)for releasing iliopsoas muscle can relieve pain of the patients,and improve their VAS score and JOA score.
