小剂量芬氟合剂结合卡前列素氨丁三醇在剖宫产术中的应用效果

摘　　要：目的探究小剂量芬氟合剂结合卡前列素氨丁三醇在剖宫产术中的应用效果。方法选取本院2018年10月到2019年10月收治的80例剖宫产术患者作为研究对象,将患者按照简单随机法分为对照组和观察组,各40例。两组患者均在胎儿娩出后使用卡前列素氨丁三醇,观察组在注射卡前列素氨丁三醇前使用芬氟合剂,对照组则注射等容量的生理盐水。比较两组患者的手术输液量、术中出血量、手术时间、不良反应(恶心、呕吐、胸闷、面部潮红、高血压、心动过速)发生情况及T1、T2、T3时的警觉与镇静(OAA/S)评分、平均动脉压(MAP)以及心率(HR)。结果观察组的手术输液量、术中出血量均少于对照组,手术时间短于对照组(P<0.05)。观察组患者用药后恶心、呕吐、胸闷、面部潮红、高血压、心动过速发生率均低于对照组(P<0.05)。T2、T3时,观察组患者的OAA/S评分均低于对照组(P<0.05)。T2、T3时,观察组患者的MAP、HR均低于对照组(P<0.05)。结论在剖宫产手术中,使用卡前列素氨丁三醇前注射小剂量芬氟合剂具有较高的安全性,可以减少胃肠道、心血管的不良反应发生情况,提高患者的治疗效果,值得推荐。Objective To explore the application effect of low dose fentanyl and droperido mixture combined with carboprost tromethamine in cesarean section.Methods Eighty patients who underwent caesarean section surgery admitted in our hospital from October 2018 to October 2019 were selected as the research objects,and the patients were divided into control group and observation group according to simple random method,with 40 cases in each group.The carboprost tromethamine was used in the both groups after delivery of fetus,the observation group received fentanyl and droperido mixture before injection carboprost tromethamine,and the control group received saline of equal volume.The operation infusion volume,intraoperative blood loss,operation time,occurrence of adverse reactions(nausea,vomiting,chest tightness,facial flushing,hypertension,tachycardia),vigilance and sedation(OAA/S)scores,mean arterial pressure(MAP)and heart rate(HR)at T1,T2,and T3 were compared between the two groups.Results The operative infusion volume and intraoperative blood loss in the observation group were less than those in the control group,and the operative time was shorter than that in the control group(P<0.05).The incidences of nausea,vomiting,chest tightness,facial flushing,hypertension and tachycardia in the observation group were all lower than those in the control group(P<0.05).At T2 and T3,OAA/S scores of patients in the observation group were lower than those in the control group(P<0.05).At T2 and T3,MAP and HR in the observation group were lower than those in the control group(P<0.05).Conclusion In cesarean section,injection low dose fentanyl and droperido mixture before the carboprost tromethamine used has higher safety,it can reduce the occurrence of adverse reactions of gastrointestinal and cardiovascular,and improve the treatment effect of patients,which is worthy recommending.
