对中医辨治新型冠状病毒肺炎的几点思考

摘　　要：认为新型冠状病毒肺炎的发病时间与气候异常相关,符合《素问》"运气学说"对"病温厉"的预判。分析庚子年(2020年)春寒气候特点、疫情及预判,并提出一般辨治方法:初期、轻症患者,予银翘散合桑菊饮加减,以宣散肺气郁热、清热化痰解毒;中、重度患者,予麻杏石甘汤加味,以宣肺平喘、清热化痰解毒,补气防脱固脱加人参;恢复期患者,予桑杏汤合玉屏风散、异功散加味,以益气养阴、健脾和胃,清解余热、余痰、余毒。除一般辨治方法外,强调遵循因时、因地、因人治则。同时对新型冠状病毒肺炎初起是否必为寒湿郁表和祛湿方法要点加以论述。It is believed that the onset time of the novel coronavirus disease(COVID-19)is related to the abnormal climate,which is in line with the prediction of doctrine of five evolutive phases and six climatic factors in Plain Questions(《素问》)for the"Binwenli"(病温厉).The spring cold weather characteristics,epidemic situation and prediction in the year 2020 was analyzed,and general methods of diagnosis and treatment of syndrome differentiation was proposed.In the early stage,for mild patients,modified Yinqiao Powder(银翘散)and Sangju Drink(桑菊饮)was used to disperse lung qi and clear stagnated heat,dissipate phlegm,remove toxicity;moderate and severe patients were given Maxing Shigan Decoction(麻杏石甘汤),to free lung and relieve asthma,to clear heat and dissipate phlegm,to remove toxicity,to supplement qi,to prevent depletion and to relieve desertion.For patients in the reco-very period,Sangxing Decoction(桑杏汤)and Yupingfeng Powder(玉屏风散),modified Yigong Powder(异功散)were given to benefit qi and nourish yin,invigorate the spleen and harmonize stomach,clear residual heat,residual phlegm and residual toxicity.In addition to the general methods of differentiation and treatment,principle of treatment in accordance with seasonal conditions,local conditions and patient′s individuality is emphasized.At the same time,discussion was made on whether the COVID-19 must be cold-damp converging in the exterior and the method of removing dampness.
