摘　　要：目的:探讨超敏C-反应蛋白及白蛋白对心力衰竭(心衰)患儿病情严重程度评估的作用。方法:分析133例心力衰竭患儿,按改良Ross评分分为轻度心衰组(3~6分)、中度心衰组(7~9分)、重度心衰组(10~12分),对患儿的血液学指标N-端脑利钠肽前体(N-terminal pro-brain natriuretic peptied,NT-proBNP)、红细胞分布宽度(red cell distribution width,RDW)、超敏C-反应蛋白(hypersensitive c-reactive protein,hs-CRP)、白蛋白(albumin,ALB)、钠离子(Na+)、钾离子(K+)进行单因素分析,然后用有序多分类logistic回归进行多因素综合分析。结果:与正常组相比,hsCRP增高组的心衰程度更高,差异有统计学意义(P<0.001,Z=-3.544)。与正常组相比,ALB降低组的心衰程度更高,差异有统计学意义(P<0.001,Z=-3.551)。结论:患儿hs-CRP愈高,ALB愈低,预后愈差。hs-CRP及ALB可评估心力衰竭患儿的病情严重程度,对评估该类患儿预后有重要临床意义。Objective To explore the effect of hypersensitive C-reactive protein and albumin on the severity of cardiac failure in children.Method A total of 133 cases of patients with cardiac failure were analyzed,and were divided into the mild cardiac failure group(3~6 points),the moderate cardiac failure group(7~9 points)and the severe cardiac failure group(10~12 points)according to the modified Ross score.Single-factor analysis was performed on the hematological indicators of the children,including N-terminal pro-brain natriuretic peptied(NT-proBNP),red cell distribution width(RDW)、hypersensitive c-reactive protein(hs-CRP),albumin,Na+and K+,and then multi-factor comprehensive analysis was conducted with orderly and multiple logistic regression.Results Compared with the normal group,the hs-CRP elevation group had a higher degree of cardiac failure(P<0.001,Z=-3.544).Cardiac failure was higher in the albumin reduced group than in the normal group(P<0.001,Z=-3.551).Conclusion The higher the hypersensitive C-reactive protein,the lower the albumin,and the worse the prognosis of the children.The hypersensitive c-reactive protein and albumin can evaluate the severity of cardiac failure in the children,which is of important clinical significance for the evaluation of the prognosis of these children.
