血必净注射液对脓毒症患者凝血功能的影响

摘　　要：目的:探讨血必净注射液对脓毒症患者凝血功能的影响。方法:选取2017年2月~2019年3月自贡市第三人民医院急诊科收治的102例脓毒症患者为研究对象。将患者随机分为观察组和对照组,各51例。对照组予以抗感染以及器官功能支持治疗;观察组在对照组基础上加用血必净注射液治疗,治疗2周。比较两组治疗前后氧化应激反应指标[活性氧(ROS)、丙二醛(MDA)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-px)、过氧化氢酶(CAT)]和炎性因子指标[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)];比较两组住ICU时间、急性生理与慢性健康状况评分(APACHEⅡ评分)及病死率。结果:治疗后,观察组血清ROS、MDA水平低于对照组,血清SOD、GSH-px、CAT水平高于对照组,差异有统计学意义(P<0.05);观察组血清TNF-a、IL-6、IL-8低于对照组,差异有统计学意义(P<0.05);观察组APACHEⅡ评分低于对照组,差异有统计学意义(P<0.05);观察组住ICU时间短和病死率低于对照组,差异有统计学意义(P<0.05)。结论:对脓毒症患者予以血必净注射液治疗可缓解氧化应激,减轻炎性反应,纠正凝血功能障碍,改善患儿的病情,缩短住ICU时间,降低病死率,值得临床推广应用。Objective To investigate the effect of Xuebijing injection on coagulation function in sepsis patients.Method 102 patients with sepsis admitted to the emergency department of the Third People’s Hospital of Zigong City from February 2017 to March 2019 were selected as the study subjects.The patients were randomly divided into observation group and control group,51 cases in each group.The control group was treated with anti-infection and organ function support,while the observation group was treated with Xuebijing injection on the basis of the control group for 2 weeks.The ROS MDA,SOD,GSH-PX,CAT and TNF-α、IL-6、IL-8 were compared between the two groups before and after treatment.The ICU stay time,APACHEⅡscore and mortality were compared between the two groups.Results After treatment,The SOD、GSH-PX and CAT in the observation group were higher than those in the control group(P<0.05).The ROS,MDA,TNF-α、IL-6 and IL-8 in the observation group were lower than those in the control group(P<0.05).The APACHEⅡscore in the observation group was lower than that in the control group(P<0.05).The ICU stay in the observation group was shorter and mortality was lower than that in the control group(P<0.05).Conclusion The treatment of Xuebijing injection for sepsis patients can correct coagulation dysfunction,improve the condition of children,shorten ICU time and reduce mortality.
