血尿酸水平、颈动脉内膜厚度与2型糖尿病中医证型的相关性研究

摘　　要：目的:研究高尿酸血症、颈动脉内中膜厚度(Intima-media Thickness,IMT)在2型糖尿病中医证候方面的分布规律、病机特点,探讨二者与中医证型的相关性.方法:搜集河南中医药大学第三附属医院内分泌科2015年8月-2016年8月242位住院患者的血尿酸水平、颈动脉厚度、体质量指数(Body Mass Index,BMI)及中医证型等数据,通过组间比较、关联规则分析及多分类Logistic回归分析等方法进行统计学分析,得出二者与2型糖尿病中医证型的相关性.结果:通过对5种中医证型的血尿酸和颈动脉内膜厚度的组间分布进行描述,并进行组间比较,结果显示组间差异有统计学意义.通过组间关联规则分析提示:内膜异常与脾虚痰湿、湿热困脾有较强的关联.通过多分类Logistic回归分析,结果显示随着血尿酸水平的降低、IMT以及BMI的升高,脾虚痰湿的优势比上升;随着血尿酸水平的降低,气阴两虚的优势比略有上升;随着IMT、餐后血糖和BMI的升高,湿热困脾的优势比上升;阴阳两虚组血尿酸水平、IMT、餐后血糖和BMI的回归系数差异均无统计学意义,提示阴阳两虚与这些变量无较强相关性.结论:血尿酸水平、颈动脉内膜厚度与2型糖尿病中医证型分布有一定的相关性.其中颈动脉内膜异常与脾虚痰湿、湿热困脾有较强的关联;血尿酸水平与脾虚痰湿、气阴两虚有一定的相关性;IMT及血尿酸水平与阴阳两虚型无明显相关性.Objective:To study the distribution and pathogenesis of hyperuricemia and carotid intima-media thickness in TCM syndromes of type 2 diabetes,and to explore the correlation between them and TCM syndromes.Methods:Collected data of blood uric acid level,carotid artery thickness,body mass index and TCM syndrome types of 242 patients.Statistical analysis was carried out by comparison of differences between groups,association rule analysis and multi-class logistic regression analysis,and the correlation between the two groups and TCM syndromes of type 2 diabetes was obtained.Results:The intergroup distribution of blood uric acid and carotid intima thickness were described by five TCM syndrome types,and the differences between the groups were compared.The results showed that the differences between the groups were statistically significant.The analysis of association rules between groups suggests that abnormal endometrium is strongly associated with spleen deficiency,Pixu Tanshi(脾虚痰湿)and Shire Kunpi(湿热困脾).Through multi-class logistic regression analysis,the results show that with the decrease of blood uric acid,IMT and BMI,the odds ratio of Pixu Tanshi increase;with the decrease of blood uric acid,the odds ratio of Qiyin Liangxu(气阴两虚)slightly increase.With the increase of IMT,postprandial blood glucose and BMI,the odds ratio of Shire Kunpi increased.The regression coefficients of blood uric acid,IMT,postprandial blood glucose and BMI in the Qiyin Liangxu groups were not statistically significant,suggesting that Qiyin Liangxu and these variables no strong correlation.Conclusion:Blood uric acid level and carotid artery intima thickness have certain correlation with the distribution of TCM syndromes of type 2 diabetes.Among them,abnormal carotid endometrium is strongly associated with Pixu Tanshi,Shire Kunpi.Blood uric acid level has a certain correlation with Pixu Tanshi,Qiyin Liangxu IMT and blood uric acid levels are not obvious with the Qiyin Liangxu type correlation.
