摘　　要：目的分析对绝经后阴道壁病变激光治疗前雌激素预处理的临床价值。方法采用前瞻性研究方法,选取2016年4月至2018年6月期间106例南京医科大学附属无锡市妇幼保健院收治的择期实施激光治疗的绝经后阴道壁病变患者为研究对象,按照随机数字表法将其分为常规组和研究组,每组各53例。常规组患者在激光治疗后给予阴道雌激素给药处理,研究组患者分别在激光治疗前和治疗后给予阴道雌激素给药处理。比较两组患者的手术治疗效果、术后康复情况及并发症发生率。结果研究组患者的治疗有效率为92.45%,较常规组(77.63%)高,差异具有统计学意义(χ^2=4.711,P<0.05);研究组患者的术后创面愈合时间[(18.69±5.36)d]、阴道流液时间[(6.35±1.20)d]均较常规组[(24.42±3.56)d、(7.43±1.58)d]短,治疗次数[(2.02±0.37)次]较常规组[(2.31±0.54)次]少,差异具有统计学意义(t=6.483、3.963、3.225,P<0.05);研究组患者的术后创面感染、创面出血发生率分别为5.66%、7.55%,较常规组(20.75%、22.64%)低,差异具有统计学意义(χ^2=5.267、4.711,P<0.05)。结论对绝经后阴道壁病变激光治疗前实施雌激素预处理,可提升手术治疗效果,提升术后康复速度,降低术后并发症发生率,效果显著。Objective To analyze the clinical value of estrogen preconditioning before laser treatment of postmenopausal vaginal wall lesions.Methods 106 postmenopausal patients with vaginal wall lesions admitted to the department of gynecology of Wuxi Maternal and Child Health Care Hospital Affiliated to Nanjing Medical University from April 2016 to June 2018 were prospectively selected as the research objects,and they were randomly divided into the conventional group and the study group,with 53 cases in each group.Patients in the conventional group were treated with vaginal estrogen after laser treatment,while patients in the study group were treated with vaginal estrogen before and after laser treatment.The laser treatment effect,postoperative recovery and incidence of complications of the two groups were compared.Results The effective rate of treatment in the study group was 92.45%,higher than that in the conventional group(77.63%)(χ^2=4.711,P<0.05).The postoperative wound healing time[(18.69±5.36)d]and vaginal fluid flow time[(6.35±1.20)d]in the study group were shorter than those in the conventional group[(24.42±3.56)d and(7.43±1.58)d],and the treatment times in the study group were(2.02±0.37),less than that in the conventional group(2.31±0.54)(t=6.483,3.963,3.225,P<0.05).The incidence of postoperative wound infection and wound bleeding in the study group were 5.66%and 7.55%,respectively,which were lower than those in the conventional group(20.75%and 22.64%)(χ^2=5.267,4.711,P<0.05).Conclusion Estrogen preconditioning before laser treatment of postmenopausal vaginal wall lesions can improve the effect of surgical treatment,postoperative recovery,and reduce the incidence of postoperative complications.
