摘  要:为探讨右美托咪定对结肠癌根治术患者围术期细胞免疫功能及血液微转移的影响,将2016年3月至2019年3月我院收治的100例择期行腹腔镜结肠癌根治术的患者按随机数字表法分为观察组和对照组,各50例,观察组麻醉诱导前静脉泵注右美托咪定负荷剂量1μg/kg,之后以0.5μg/(kg·h)的速度持续静脉输注至手术结束;对照组接受等量等速的0.9%氯化钠注射液。比较2组患者麻醉诱导前(T0)、手术开始1h时(T1)、手术完成即刻(T2)、术后24h(T3)时T淋巴细胞亚群CD3^+、CD4^+、CD8^+及NK细胞水平,比较2组患者T0、T3时细胞角蛋白20(CK20)mRNA阳性表达水平。结果显示,对照组患者T2、T3时CD3^+、CD4^+、NK细胞水平较T0时明显降低,P<0.05;观察组患者T2、T3时CD3^+、CD4^+细胞水平较T0时明显降低,P<0.05。观察组患者T2、T3时CD3^+、CD4^+、NK明显高于对照组,P<0.05。2组患者T0时血CK20mRNA阳性率比较差异无统计学意义,P>0.05;T3时观察组患者血CK20mRNA阳性率明显低于对照组,P<0.05。结果表明,右美托咪定可改善结肠癌根治术患者围术期细胞免疫功能,降低血液微转移率。This study was to investigate the influence of Dexmedetomidine(DEX)on the cellular immunity function(CIF)and micro hemometastasis at perioperation of the colonic cancer patients undergoing scheduled radical procedure,randomly divided 100 patients to be subject to laparoscopic radical procedure in author’s hospital(2016-03-2019-03)into observation group and control group,50 cases for each;the former was injected intervenously by pamp DEX load dosage(1μg/kg)before anasthesia revulsion,then in velocity of 0.5μg/(kg·h)lasting to the end of operation,meanwhile,in the latter,only 0.9%NaCl injection as same dosage and velocity as the former;Then,compared both groups’T-cell subsets(CD3^+,CD4^+,CD8^+)and NK cell’s level before anesthesia induction(T0),1 hat the beginning of operation(T1),inmediately after operation(T2),and 24 hafter operation(T3),also compared both groups’expression level of cytokeratin 20(CK 20)mRNA positive rate at T0,T3.As results,in the latter the CD3^+,CD4^+and NK cell’s level at T2 and T3 were significantly decreased than that at T0(P<0.05),but in the former,only CD3^+and CD4^+levels did so(P<0.05);Also,in the former patients’CD3^+,CD4^+and NK cells levels at T2 and T3 were significantly higher than that in the latter(P<0.05);at T0 in CK20 mRNA positive rate there was no statistical difference between the two groups(P>0.05),but at T3 the former was significantly lower(P<0.05).Results show that DEX can improve cellular immuno-function and reduce micro hemometastasis at perioperation of the patients undergoing radical surgery for colonic cancer.
