摘　　要：慢性阻塞性肺疾病(COPD)已被证实是一种多基因遗传的慢性疾病,其中一个严重的并发症是肺动脉高压(PH),高达30%的患者病程中伴有PH,严重增加了死亡率。目前,COPD相关的肺动脉高压(COPD-PH)发病机制仍不清楚,临床上以对症治疗为主。早期已有证据表明遗传因素在PH疾病的发展中起重要作用。近年来,随着遗传学研究技术的进步,发现不少基因的多态性与COPD-PH进展相关,加深了对COPD-PH遗传机制的认识,也为个体化诊疗提供了新的可能。本综述复习整理了相关的基础研究和临床观察,探讨了COPD-PH遗传学机制的研究进展。Chronic obstructive pulmonary disease(COPD)has been proved to be a polygenic chronic disease.One of the serious complications is pulmonary hypertension(PH).Up to 30%of patients have PH during the course of disease,which significantly increases mortality.At present,the pathogenesis of pulmonary hypertension associated with COPD(COPD-PH)remains unclear,and clinical symptomatic treatment is the mainstay.Early evidence has shown that genetic factors play an important role in the development of PH.In recent years,with the advancement of genetics research technology,it has been found that many genes polymorphism are associated with the progression of COPD-PH,which has deepened the understanding of the genetic mechanism of COPD-PH and also provides new possibilities for individualized diagnosis and treatment.This article reviews the related basic research and clinical observation,and investigate the progress of the genetic mechanism of COPD-PH.
