胰岛素泵皮下注射和多次胰岛素皮下注射治疗妊娠期糖尿病疗效的比较

摘　　要：目的讨论妊娠期糖尿病(GDM)采用胰岛素泵皮下注射和多次胰岛素皮下注射的临床效果。方法选取2017年1月~2019年8月我院收纳GDM患者92例,按随机数字表法将其中46例纳进对照组(按多次胰岛素注射),余46例纳进实验组(按胰岛素泵注射),比较两组治疗效果。结果两组治疗前FPG、2 hPBG血糖指标比较无差异(P>0.05),治疗后实验组血糖指标明显低于对照组(P<0.05);实验组治疗总有效率为97.83%,对照组为78.26%,实验组总有效率明显高于对照组(P<0.05);两组早产、妊娠高血压、剖宫产比较无统计学意义(P>0.05),但实验组新生儿低血糖、巨大儿、胎儿窘迫明显低于对照组(P<0.05)。结论GDM患者采用胰岛素泵皮下注射效果显著,有效提高治疗效果,且降低不良症状发生,有效控制血糖指标,因此值得临床应用及推广。Objective To discuss the clinical effect of insulin pump subcutaneous injection and multiple insulin subcutaneous injections in gestational diabetes mellitus(GDM).Methods From January 2017 to August 2019,92 patients with GDM were admitted to our hospital.According to the random number table method,46 of them were included in the control group(given multiple insulin injections),and the remaining 46 were included in the experimental group(injected by insulin pump).The treatment value was compared between the two groups.Results There was no difference in blood glucose indices of FPG and 2 hPBG before treatment between the two groups(P>0.05).After treatment,the blood glucose index in the experimental group was significantly lower than that in the control group(P<0.05);the total effective rate in the experimental group was 97.83%,and the control group was 78.26%.The total effective rate in the experimental group was significantly higher than that in the control group(P<0.05);there was no statistically significant difference in premature delivery,pregnancy-induced hypertension and cesarean section between the two groups(P>0.05).However,the hypoglycemia,macrosomia and fetal distress in the experimental group were significantly lower than those in the control group(P<0.05).Conclusion The subcutaneous injection of insulin pump in patients with GDM has a significant effect,which effectively improves the treatment effect,reduces the occurrence of adverse symptoms,and effectively controls blood glucose indices,so it is worthy of clinical application and promotion.
