门诊中药房饮片调配中中药处方点评对用药差错的干预效果分析

摘　　要：目的:研究分析门诊中药饮片调配中中药处方点评对用药差错的干预效果.方法:随机抽取2017年10月-2018年10月英德市中医院门诊开具的中药处方单1000张,其中前6个月476张处方单未应用中药处方点评,后6个月应用中药处方点评后开具的中药处方单为524张,比较分析中药处方点评应用前及应用后中药饮片调配的差错率及具体差错类型.结果:经组间比较显示中药处方点评应用后中药饮片调配的差错率显著低于应用前(P<0.05).且造成英德市中医院中药饮片调配差错的主要原因为调配剂量不准确、组方结构混乱、药物误配漏配情况严重、处方开具不规范、配伍禁忌等.结论:在门诊中药饮片调配中应用中药处方点评可有效降低用药差错率,保障用药安全;且相关药剂人员在调配工作过程中应明确中药饮片调配是一项长期工作,应不断增强自身防范意识,提高中药饮片调配质量,进一步确保临床疗效.Objective:To study and analyze the effect of TCM prescription comments on medication errors in the dispensing of TCM decoction pieces in outpatient service.Methods:From October 2017 to October 2018,a total of 1000 prescriptions in Yingde Hospital of Traditional Chinese Medicine were randomly selected,of which 476 prescriptions in the first 6 months were not evaluated by traditional Chinese medicine prescriptions,and 524 prescriptions were issued after the evaluation of traditional Chinese medicine prescriptions.The error rate and specific error types in the preparation of prepared slices of TCM before and after the application of TCM prescription comments were compared and analyzed.Results:The comparison between the two groups showed that the error rate of TCM decoction pieces preparation was significantly lower after the application of TCM prescription review than that before the application(P<0.05).The main reasons for the errors in the preparation of Chinese herbal decoction pieces are the inaccuracy in the preparation of dosage,the confusion in the composition of prescriptions,the serious misallocation and leakage of drugs,the non-standard prescription,and the compatibility taboos,etc.Conclusion:The application of TCM prescription comment in the dispensing of TCM decoction pieces in outpatient service can effectively reduce the error rate of medication and guarantee the medication safety.In addition,relevant pharmaceutical personnel should make it clear that the preparation of Chinese herbal decoction pieces is a long-term work,and constantly enhance their awareness of prevention,improve the quality of the preparation of Chinese herbal decoction pieces,and further ensure the clinical efficacy.
