摘　　要：目的探讨24 h血压动态监测仪在冠心病合并高血压患者治疗中的临床效果。方法于2017年1月~2019年10月选择我院80例冠心病合并高血压患者与80例单纯高血压患者,分别设置为观察组、对照组,采用24 h动态血压监测仪对患者进行动态血压监测,比较两组动态血压监测结果。依据随机数字表法将冠心病合并高血压患者分为两组,A组实施常规降血压药物治疗,B组根据24 h动态血压监测结果指导用药,比较两组的血压水平、血压变异性指标。结果相比于对照组,观察组的平均收缩压、平均舒张压、脉压差、晨峰血压均更高(P<0.05),观察组的夜间血压下降幅度更小(P<0.05)。治疗后,B组的收缩压、舒张压、收缩压变异指数与A组相比均更低(P<0.05)。结论24 h动态血压监测仪用于冠心病合并高血压患者治疗中,可反映患者血压波动情况,还可指导患者用药,有利于提高治疗效果。Objective To investigate the clinical effect of 24 h ambulatory blood pressure monitor in the treatment of the patients with coronary heart disease(CHD)complicated with hypertension.Methods From January 2017 to October 2019,80 patients with CHD complicated with hypertension and 80 patients with hypertension alone were selected and respectively taken as the observation group and the control group.24 h ambulatory blood pressure monitor was used to monitor the patients’ambulatory blood pressure(ABP),and the monitoring results of the two groups were compared.The patients with CHD complicated with hypertension were divided into two groups according to the random number table method.Group A was treated with conventional antihypertensive drugs,while Group B was treated with drugs according to 24 h ambulatory blood pressure monitoring results.The blood pressure(BP)levels and BP variability(BPV)indexes of the two groups were compared.Results Compared with those of the control group,the mean systolic pressure(mSP),mean diastolic pressure(mDP),pulse pressure difference and morning BP surge of the observation group were higher(P<0.05),while the nighttime BP decrease rate of the observation group was lower(P<0.05).After treatment,the systolic pressure,diastolic pressure and systolic pressure variability index of group B were all lower than those of group A(P<0.05).Conclusion In the treatment of the patients with CHD complicated with hypertension,24 h ambulatory blood pressure monitor can reflect the fluctuation of BP in the patients and also guide patients in taking drugs.It is beneficial to the promotion of the curative effect.
