滤纸干血片样品用于自愿咨询检测HIV抗体的研究

摘　　要：目的研究在自愿咨询检测HIV抗体工作中,用采集末梢血制成滤纸干血片替代采集静脉血的可行性。方法采集200例自愿咨询受检者的手指末梢血及静脉血(抗凝全血和非抗凝血)。将手指末梢血和静脉抗凝全血分别制成滤纸干血片样品,非抗凝血则分离出血清,并在不同条件下保存。用ELISA法和免疫印迹法检测HIV抗体,将检测结果进行比较。结果各类滤纸干血片与静脉血清经ELISA法检测,定性结果一致。与血清的测量结果相比,直径6 mm的滤纸干血片OD值有显著性差异(P<0.05)。直径10 mm的滤纸干血片OD值无显著性差异(P>0.05),滤纸干血片样品用生理盐水和pH7.4的PBS洗脱均可,300μL的生理盐水洗脱效果更好。滤纸干血片保存1周的结果稳定。用免疫印迹法检测,滤纸干血片与静脉血清判读结论一致。结论采集末梢血制成的滤纸干血片,以ELISA法检测HIV抗体,结果可靠、稳定,且操作简便,运输和保存无生物危害,可替代采集静脉血用于自愿咨询HIV抗体检测工作。Objective To study the feasibility of using collected peripheral blood to make dried-blood spots(DBSs)on filter paper instead of collecting venous blood in voluntary consultation on HIV antibody detection.Methods The finger peripheral blood and venous blood(anticoagulant whole blood and non-anticoagulant blood)of 200 consultation and detection volunteers were collected.Finger peripheral blood and venous anticoagulant whole blood(WB)were respectively made into dried-blood spot specimens on filter paper,while serum was disassociated from non-anticoagulant blood,and they were both stored under different conditions.The HIV antibody was detected by enzyme-linked immunosorbent assay(ELISA)and immunoblotting test(IBT),and the detection results were compared.Results When all kinds of DBSs on filter paper and venous serum were detected by ELISA,the qualitative results were consistent.Compared with the detection results of serum,the OD values of DBSs on filter paper with a diameter of 6 mm were significantly different(P<0.05).There was no significant difference in the OD values of DBSs on filter paper with a diameter of 10 mm(P>0.05).The dried-blood spot specimens on filter paper could be eluted with normal saline and PBS with pH 7.4,and the elution effect of 300μL normal saline was better.The results of one week storage of DBSs on filter paper were stable.When IBT was used for detection,the interpretation conclusions of DBSs on filter paper and venous serum were consistent.Conclusion When ELISA is used to detect the HIV antibody in DBSs on filter paper made by collecting peripheral blood,the results are reliable and stable,the operation is simple and convenient,and the transportation and storage cause no biological hazards.It can be applied to voluntary consultation on HIV antibody detection as a substitute for collecting venous blood.
