应用血清β-HCG增长率、孕酮值及子宫内膜厚度对鉴别不明部位妊娠的价值

摘　　要：目的探讨应用人绒毛膜促性腺激素(β-HCG)、孕酮和子宫内膜厚度在预测不明部位妊娠中的价值。方法回顾性分析208例因诊断"不明部位妊娠"入院,之后被证实为异位妊娠和宫内妊娠的患者资料,分析应用血清β-HCG增长率、孕酮和子宫内膜厚度对不明部位妊娠的预测价值,分析确定各指标单独应用时的预测界值并评估敏感度和特异度。结果异位妊娠患者中,血β-HCG增长率,孕酮及子宫内膜厚度均低于宫内妊娠者,多因素分析提示使用血清β-HCG增长率和孕酮对异位妊娠的预测均有统计学意义,比较其预测价值排序为:孕酮值>β-HCG增长率>子宫内膜厚度。再分别计算各指标的预测界值,结果提示孕酮的预测界值为18.48 nmol/L,敏感度为72.7%,特异度为92.6%;β-HCG增长率预测异位妊娠的界值为81.77%,其敏感度为81.8%,特异度为75.9%。结论使用血清β-HCG增长率和孕酮对不明部位妊娠进行预测,具有临床应用价值并值得推广。Objective To explore the value of human chorionic gonadotropin(β-HCG),progesterone(P) and endometrial thickness in differentiating pregnancy of unknown location.Methods The data of 208 patients admitted to hospital for diagnosis of "pregnancy of unknown location,PUL" were retrospectively analyzed.Analyze the predictive value of serum β-HCG growth rate,progesterone and endometrial thickness for pregnancy of unknown location,and to determine the indicators.It was analyzed and prediction thresholds for single application and evaluation of sensitivity and specificity.Results The growth rate of serum β-HCG,progesterone and the thickness of and endometrium in patients with ectopic pregnancy were lower than those in patients with intrauterine pregnancy.Multivariate analysis showed that the growth rate of serum β-HCG and progesterone there were statistical significantly differences in predicting ectopic pregnancy.The order of predictive value was progesterone >the growth rate of serum β-HCG>endometrial thickness.The results showed that the predictive threshold of progesterone was 18.48 nmol/L,the sensitivity was 72.7%,and the specificity was 92.6%.The predictive threshold of β-HCG growth rate for ectopic pregnancy was 81.77%,the sensitivity was 81.8%,and the specificity was 75.9%.Conclusion The use of serum β-HCG growth rate and progesterone to predict PUL has clinical value and is worth promoting.
