拔火罐对3种毒蛇咬伤患者局部症状的改善作用

摘　　要：目的:探讨拔罐疗法对3种血循毒蛇咬伤患者局部症状的改善作用。方法:选取126例蝰蛇、竹叶青蛇和眼镜蛇咬伤患者按不同蛇种随机分为两组,对照组采用常规的治疗和护理,观察组在对照组医护基础上于蛇咬伤处或肿胀部位拔火罐,以治疗前后患肢肿胀程度、患肢疼痛程度和炎症指标变化来评价治疗与护理效果。结果:(1)患肢肿胀程度:3种毒蛇咬伤患者在治疗后第3天观察组明显小于对照组(P<0.05),蝰蛇伤观察组在治疗后第3天明显小于第1天(P<0.01),竹叶青蛇伤观察组在治疗后第3天明显小于第1天和治疗前(P<0.01),眼镜蛇伤观察组在治疗后第3天明显小于治疗前(P<0.01)。(2)患肢疼痛程度:竹叶青蛇伤在治疗后第3天观察组比对照组明显减轻(P<0.05);3种毒蛇咬伤患者治疗后第1天和第3天疼痛评分在两组均呈进行性降低,第3天降低更明显(P<0.05或P<0.01)。(3)炎症指标:治疗后第1天蝰蛇伤对照组的超敏C反应蛋白(High Sensitivity C-reactive Protein,hs-CRP)水平比观察组明显升高(P<0.05)、竹叶青蛇和眼镜蛇伤对照组的血清淀粉样蛋白A(Serum Amyloid A,SAA)和hs-CRP水平均比观察组明显升高(P<0.05),观察组在3种毒蛇咬伤患者的hs-CRP水平及眼镜蛇伤的白细胞总数(White Blood Cell,WBC)水平在治疗后第1天与治疗前比较均无明显升高(P>0.05)。结论:拔罐疗法能使血循毒蛇咬伤患者有效消除患肢肿胀,减轻患肢疼痛,并能改善炎症状况。Objective:To probe into the improvement of cupping therapy for the local symptoms in the three kinds of snake bite patients of circulating toxin.Methods:126 cases were respectively divided into two groups according to the snake species.The control group was given the routine treating and nursing methods.The observation group was given cupping in the position of bite or the location of swelling more.Results:The swelling degree of ill limb in the three kinds of snake bite patients of the observation group was apparently lower than the control group(P<0.05),that in the viper bite patients of the observation group in the third day was apparently lower than first days(P<0.01),that in the trimeresurus gramineus bite patients of the observation group in the third day was apparently lower than first days and before treatment(P<0.01),that in the cobra bite patients of the observation group in the third day was apparently lower than before treatment(P<0.01).The pain degree of ill limb in the trimeresurus gramineus bite patients of the observation group in the third day was reduced than the control group,that in the the three kinds of snake bite patients of the observation group was reduced than the control group(P<0.05 or P<0.01).In the first day,the hs-CRP in the viper bite patients of the control group was higher than the observation group in the(P<0.05),SAA and hs-CRP in trimeresurus gramineus and cobra bite patients of the control group was apparently higher than the observation group(P<0.05).There was no significant difference of hs-CRP level in three kinds of snake bite patients and WBC in cobra bite of the observation group(P>0.05).Conclusion:The cupping therapy can efficiently eliminate the swelling of ill limb and lighten the pain of ill limb and improve the inflammatory condition for the snake bite patients of circulating toxin.
