摘　　要：目的:评价针刺远近穴位对痛证疗效的影响。方法:根据循证医学(EBM)及系统评价相关方法,对近20年国内外常用数据库针刺远近穴位治疗痛证的文献进行评价研究。结果:纳入研究11个患者653例,Meta分析显示远近取穴针刺1次,疼痛疗效比较PPI量表和VAS量表,2组间差异有统计学意义,表明针刺1次远端取穴疗效优于近端取穴,远近取穴针刺1个疗程、2个疗程和2个疗程以上时2组间比较差异无统计学意义,表明针刺1个疗程或更长时间远近取穴疗效相当。结论:快速镇痛应选择针刺远端穴位,需长期镇痛治疗时针刺远近端穴位疗效相差不大。Objective:To systematically evaluate the effect of acupuncture at far or near acupoints for the treatment of pain.Methods:According to the requirements of evidence-based medicine(EBM)and the systematic evaluation manual method,the literature about the effect of acupuncture at far and near acupoints on the curative effect of pain was searched and evaluated in domestic and international databases in the past 20 years.Results:A total of 11 studies were included,with a total of 653 patients.The results of the Meta analysis are as follows.The comparison of the effect of PPI and VAS on the curative effect of acupuncture at the far or near acupoints showed that there were a statistically significant difference between the two groups in the effect of acupuncture on pain when patients were given treatment only one time.It could be considered that the analgesic effect of acupuncture at the distal point was better than that of the proximal acupoint when patients were given treatment only one time;the comparison of the curative effect of acupuncture at the far or near acupoints showed that there were no statistically significant difference between the two groups in the effect of acupuncture on pain when patients were given treatment one course、two courses and more than two courses.It could be considered that the analgesic effect of acupuncture at the distal acupoint was the same of the proximal acupoint when patients were given treatment one or more course.Conclusion:Distal acupuncture has a faster therapeutic effect on disease.For long term(one course or above)acupuncture treatment,there is little difference between distal and proximal acupoint selection.
