口服黄体酮胶囊联合雌激素注射治疗子宫异常出血疗效

摘　　要：目的:探究黄体酮胶囊联合肌内注射雌激素治疗子宫异常出血效果。方法:选取本院妇科2017年6月-2018年6月就诊的功能性子宫异常出血患者96例,随机分为对照组和观察组各48例,对照组采取雌激素注射治疗,观察组在对照组治疗基础上联合口服黄体酮胶囊治疗。比较并分析治疗前后两组患者子宫内膜、月经、血红蛋白(Hb)和止血等变化。结果:较治疗前比较,治疗后两组子宫内膜厚度、月经失血图(PBAC)评分及月经期均下降,Hb水平及月经周期增加(P<0.05),且观察组子宫内膜厚度(5.26±1.45 mm)、月经PBAC评分(53.05±10.84分)、Hb水平(117.72±6.63g/L)、月经期(6.51±1.52 d)、月经周期(28.63±1.65 d)、控制出血时间(26.34±6.56 h)和完全出血时间(38.82±5.11h)均优于对照组,治疗总有效率(97.9%)高于对照组(85.4%)(均P<0.05)。结论:口服黄体酮胶囊联合雌激素注射治疗功能性子宫异常出血疗效更佳,可明显缩短出血时间,促进愈后。Objective:To explore the effect of progesterone capsule combined with estrogen intramuscular injection for treating women with abnormal uterine bleeding.Methods:96 women with the dysfunctional abnormal uterine bleeding were selected as research objects and were randomly divided into two groups from June 2017 to June 2018.48 women in the control group were treated by estrogen intramuscular injection,and 48 women in the observation group were treated by progesterone capsule combined with estrogen intramuscular injection.Endometrial thickness,menstruation situation,hemoglobin(Hb)level,and hemostasis effct of women before and after treatment were compared between the two groups.Results:After treatment,the endometrial thickness,the menstrual PBAC score,and the menstrual duration of women in both groups had decreased significantly,however,the Hb level and the menstrual cycle time of women in both groups had increased significantly(P<0.05).The endometrial thickness,menstrual PBAC score,Hb level,menstrual duration,menstrual cycle time,time of bleeding controlled,and bleeding time of women in the observation group were 5.26±1.45 mm,53.05±10.84,117.72±6.63 g/L,6.51±1.52 d,28.63±1.65 d,26.34±6.56 h,and 38.82±5.11 h,respectively,which were all significant better than those of women in the control group(P<0.05).Meanwhile,the total effective rate of women in the observation group was 97.9%,which was significant higher than that(85.4%)of women in the control group(P<0.05).Conclusion:The treatment of progesterone capsule combined with estrogen injection for treating women with dysfunctional abnormal uterine bleeding has significant effective,which can shorten bleeding duration and improve prognosis.
