经阴道超声联合血清β-hCG诊断异位妊娠临床价值

摘　　要：目的:探讨经阴道超声联合血清绒毛膜促性腺激素(β-hCG)诊断异位妊娠临床价值。方法:选取本院经腹腔镜手术证实为异位妊娠56例,另选正常宫内妊娠要求终止妊娠50例为早孕组,回顾性分析两组一般资料,结合经阴道超声及血清β-hCG检测结果,比较两组血β-hCG及子宫内膜厚度并绘制ROC曲线,观察诊断异位妊娠价值。结果:异位妊娠组血清β-hCG及子宫内膜厚度均低于早孕组(P<0.05)。ROC曲线显示血清β-hCG诊断异位妊娠的最佳截断值为622.5 IU/L,此时敏感度0.889%,特异度0.910%;子宫内膜厚度诊断异位妊娠最佳截断值8.05 mm,敏感度0.960%,特异度0.893%;结合ROC结果,分别将血清β-hCG和子宫内膜厚度临界值作为阳性,kappa值血清β-hCG为0.792,子宫内膜厚度为0.848。结论:经阴道超声检测子宫内膜联合血清β-hCG水平诊断异位妊娠有一定临床参考价值。Objective:To explore the clinical value of transvaginal ultrasound combined with serum beta-hCG(β-hCG)level for diagnosing ectopic pregnancy.Methods:56 women with ectopic pregnancy confirmed by laparoscopic operation were included in group A,and another 50 women with normal unwanted intrauterine pregnancy were selected in group B.The general data of women in the two groups were analyzed retrospectively.The serumβ-hCG level and endometrial thickness of women were compared between the two groups and ROC curve were analyzed based on the results of transvaginal ultrasound examination combined with serumβ-hCG level.The value of transvaginal ultrasound combined with serumβ-hCG level for diagnosing ectopic pregnancy was observed.Results:The serumβ-hCG level and endometrial thickness of women in group A were significant lower than those of women in group B(P<0.05).ROC curve showed that the best cut-off value of serumβ-hCG level for diagnosing ectopic pregnancy was 622.5 IU/L,and its sensitivity and specificity were 0.889 and 0.910.Endometrial thickness for diagnosing ectopic pregnancy was 8.05 mm,and its sensitivity and specificity were 0.960 and 0.893.According to ROC results,when the cut-off value ofβ-hCG level or endometrial thickness was regarded as positive,the kappa of serumβ-hCG level and endometrial thickness were 0.792 and 0.848.Conclusion:Endometrial thickness by transvaginal ultrasound combined with serumβ-hCG level for diagnosing ectopic pregnancy has certain reference value.
