摘  要:目的三腔胃肠管在上消化道穿孔早期肠内营养中的应用。方法将来该院的78例上消化道穿孔患者作为研究对象,研究时间为2015年3月—2019年3月,根据患者入院顺序将其分为两组,即研究组(n=39)与对照组(n=39),对照组患者给予常规肠内营养干预,研究组患者给予三腔胃肠管联合早期肠内营养干预,干预后评价干预效果、不良反应发生率、患者营养状况及生活质量。结果干预效果:研究组住院时间(9.68±1.25)d及消耗费用(1.98±0.89)万元相较对照组明显偏低(P<0.05)。不良反应发生率:研究组不良反应发生率为5.12%,对照组不良反应发生率为23.07%,研究组不良反应发生率明显低于对照组(χ~2=5.18,P=0.02)。营养状况:干预前,研究组患者营养状况相较对照组差异无统计学意义(P>0.05);干预后,研究组营养状况(99.89±8.56)g/L、(34.18±6.53)g/L、(65.78±5.12)g/L明显优于对照组(P<0.05)。结论三腔胃肠管在上消化道穿孔早期肠内营养中具有显著的价值,既能缩短患者住院时间,减少患者消耗费用,又能降低不良反应发生率,改善患者营养状况。Objective The application of three-lumen gastrointestinal tube in early enteral nutrition in upper gastrointestinal perforation.Methods In the future,78 patients with upper gastrointestinal perforation in the hospital will be taken as the research object.The study started in March 2015 and ended in March 2019.They were divided into two groups according to the order of patients'admission,namely the study group(n=39)and the control group(n=39),the patients in the control group were given conventional enteral nutrition intervention,and the patients in the study group were given three-lumen gastrointestinal tube combined with early enteral nutrition intervention,and the intervention effect,adverse reaction rate,and patient nutrition were evaluated after the intervention condition and quality of life.Results Intervention effect:The length of hospital stay(9.68±1.25)d and expenditure(1.98±0.89)ten thous and yuan in the study group were significantly lower than those in the control group(P<0.05).Incidence of adverse reactions:the incidence of adverse reactions in the study group was 5.12%,and the incidence of adverse reactions in the control group was 23.07%.The incidence of adverse reactions in the study group was significantly lower than that in the control group(t=5.18,P=0.02).Nutritional status:before the intervention,there was no statistically significant difference between the nutritional status of the study group and the control group(P>0.05).After the intervention,the nutritional status of the study group(99.89±8.56)g/L,(34.18±6.53)g/L,(65.78±5.12)g/L,significantly better than that of the control group(P<0.05).Conclusion The three-lumen gastrointestinal tube has significant value in early enteral nutrition in the upper gastrointestinal perforation.It can shorten the length of hospital stay,reduce the cost of patients,reduce the incidence of adverse reactions,and improve the nutritional status of patients.
