心脏声学造影在心脏病的诊断价值分析

摘　　要：目的分析探讨心脏声学造影诊断心脏病的应用价值。方法选取梅州市人民医院2018年9月—2020年3月收治的65例疑似心脏病患者进行此次研究,所有患者先后行经胸超声心动图(UCG)检查和心脏声学造影检查,通过与UCG检查结果对比,计算心脏声学造影诊断心脏病的敏感度、特异度及诊断符合率。结果UCG检查结果为阳性30例,阴性35例,心脏声学造影检查结果为阳性26例,阴性39例,与UCG检查结果对比,心脏声学造影诊断心脏病的敏感性为86.67%(26/30),特异性为111.43%(39/35),诊断符合率为93.85%(61/65);两种诊断方式在左室收缩末期容积(LVESV)和舒张末期容积(LVEDV)、左室射血分数(LVEF)及左室每搏输出量(LVSV)等指标检查结果对比上差异有统计学意义(t=8.810,P=0.000;t=2.011,P=0.046;t=6.057,P=0.000;t=2.197,P=0.030)。结论心脏声学造影在诊断心脏病有效性方面与UCG检查结果相差无几,但对心肌灌注情况及右向左分流的诊断更加准确,同时对心功能指标的评估更加准确,为心脏病的早期诊断和治疗提供方案,值得推广。Objective To analyze and probe into the application value of cardiac contrast echocardiography in diagnosis of heart disease.Methods 65 cases of patients who suspected heart disease and who treated in Meizhou people’s Hospital from September 2018 to March 2020 were selected in this study,and all patients underwent transthoracic echocardiography(UCG)and cardiac contrast echocar-diography.Then,the sensitivity,specificity and coincidence rate of contrast echocardiography were calculated by comparing with UCG.Results The results of UCG examination was positive in 30 cases and negative in 35 cases,and the results of cardiac contrast echocar-diography were positive in 26 cases and negative in 39 cases.Compared with the results of UCG,the sensitivity and specificity of cardiac contrast echocardiography were 86.67%(26/30),111.43%(39/35)and 93.85%(61/65)respectively;There were significant differences between the two diagnostic methods in terms of left ventricular end systolic volume(LVESV),end diastolic volume(LVEDV),left ventricular ejection fraction(LVEF)and left ventricular stroke output(lvsv)(t=8.810,P=0.000;t=2.011,P=0.046;t=6.057,P=0.000;t=2.197,P=0.030).Conclusion The effectiveness of cardiac contrast echocardiography in diagnosis of heart disease are similar to those of UCG,but the diagnosis of myocardial perfusion and right to left shunt is more accurate,and the evaluation of cardiac function index is more accurate.It provides a scheme for the early diagnosis and treatment of heart disease,thus it is worth promoting.
