基于“健脾调肺”理论探究结肠腺瘤围手术期症状的干预治疗

摘　　要：目的:基于"肺与大肠相表里"的中医理论及前期研究,将健脾调肺法应用于结肠腺瘤围手术期症状的干预治疗。选用江苏省中医院院内制剂肠安胶囊,相对于汤剂,该药服用方法便捷,既往多用于溃疡性结肠炎患者的相关治疗,并取得一定疗效。结合前期相关基础及临床研究,期望为围手术期肠功能紊乱的患者提供安全有效、快速康复的治疗策略。方法:将60例围手术期出现肠功能紊乱的患者随机分为对照组、治疗组,每组各30例。两组均行内镜下切除术,术后治疗组予双歧杆菌三联活菌胶囊(培菲康)+肠安胶囊口服,疗程4周,对照组予双歧杆菌三联活菌胶囊,疗程4周。观察两组临床疗效、中医证候积分及术后并发症等情况。结果:治疗组临床综合疗效优于对照组(P<0.05);在中医证候积分中部分症状改善方面,治疗组优于对照组(P<0.05);两组均有1例术后出血;未发生严重感染及迟发性穿孔等并发症。结论:肠安胶囊可以安全有效地改善患者围手术期临床症状。Objective:Based on the TCM theory of interior-exterior of the lung and the large intestine,and the previous studies.This topic aims to observe clinical efficacy of harmonizing lung and spleen was applied to perioperative symptoms after endoscopic treatment of colonic adenoma.Chang-an capsule was selected from Jiangsu hospital of Traditional Chinese Medicine.Compared with decoction,this medicine is convenient to take.In the past,this medicine was mainly used in the treatment of ulcerative colitis patients and achieved a certain effect.Based on the previous clinical studies and the theory of traditional Chinese medicine,this paper is expected to provide safe,effective strategies for the patients with perioperative intestinal dysfunction.Methods:60 patients diagnosed with perioperative intestinal dysfunction were divided into treatment group and control group.There were 30 cases in each group.Endoscopic resection was performed in both groups.In treatment group,bifidobacterium triplex live bacteria capsules(peifeikang)+Chang An capsules were given orally for a course of 4 weeks.The control group was treated with bifidobacterium triplex live bacteria capsules for 4 weeks.The clinical efficacy,TCM syndrome score and postoperative complications of the two groups were observed.Results:The comprehensive clinical effect of the treatment group was better than that of the control group(P<0.05);In TCM syndrome score,The treatment group was superior to the control group(P<0.05);Postoperative hemorrhage occurred in 1 patient in both groups.No serious infection or delayed perforation occurred.Conclusion:Chang-an capsule can improve the perioperative clinical symptoms safely and effectively.
