鼻中隔矫正联合低温等离子射频消融对中重度变应性鼻炎伴鼻中隔偏曲患者炎性因子、免疫功能的影响

摘　　要：目的探讨鼻中隔矫正联合低温等离子射频消融对中重度变应性鼻炎伴鼻中隔偏曲患者炎性因子、免疫功能的影响。方法回顾性分析2017年6月至2018年6月在南京中医药大学常熟附属医院耳鼻咽喉科接受治疗的74例中重度变应性鼻炎伴鼻中隔偏曲患者的临床资料,按手术方法不同分为对照组和试验组,各37例。对照组患者采用鼻中隔矫正术治疗,试验组患者采用低温等离子射频消融联合鼻中隔矫正术治疗。比较两组患者术后12个月的临床疗效;比较两组患者术前及术后3、12个月炎性因子水平;比较两组患者术前、术后12个月免疫功能指标、鼻阻力、糖精清除时间、症状积分;比较两组患者术后12个月内并发症发生情况。结果术后12个月,试验组患者总有效率显著高于对照组;与术前比,术后3~12个月两组患者血清TNF-α、IL-6、CRP水平呈显著降低趋势,试验组显著低于对照组;与术前比,术后12个月两组患者CD3^+、CD4^+比例及试验组患者CD3^+/CD4^+比值均呈显著升高趋势,试验组高于对照组;与术前比,术后12个月两组患者鼻阻力显著降低,试验组显著低于对照组;与术前比,术后12个月两组患者糖精清除时间显著缩短,试验组显著短于对照组;与术前比,术后12个月两组患者流涕、鼻痒、打喷嚏、鼻塞症状积分均显著降低,试验组显著低于对照组;术后12个月内,试验组患者并发症总发生率显著低于对照组(均P<0.05)。结论鼻中隔矫正联合低温等离子射频消融可降低中重度变应性鼻炎伴鼻中隔偏曲患者血清TNF-α、IL-6、CRP水平,改善炎性症状,促进免疫功能恢复,且安全性高,疗效显著。Objective To explore the effects of septal correction combined with low temperature plasma radiofrequency ablation on inflammatory factors, immune function in patients with moderate to severe allergic rhinitis with septal deviation. Methods Retrospective analysis was performed on the clinical data of 74 patients with moderate to severe allergic rhinitis with septal deviation who were treated in the Department of Otorhinolaryngology, Changshu Affiliated Hospital of Nanjing University of Chinese Medicine from June 2017 to June 2018. According to different surgical methods, they were divided into control group and experimental group, 37 cases each. The control group was treated with septal correction, and the experimental group was treated with septal correction combined with low temperature plasma radiofrequency ablation. The clinical efficacy of the two groups were compared 12 months after operation. The levels of inflammatory factors were compared between the two groups at before operation and 3~12 months after operation. Immune function indicators, nasal resistance, saccharin clearance time, and symptom scores were compared between the two groups before operation and 12 months after operation. The occurrence of complications anong 12 months after operation were compared between the two groups. Results 12 months after operation, the total effective rate of the experimental group was significantly higher than that of the control group;compared with the preoperative level, the serum TNF-α, IL-6, CRP in the two groups significantly decreased 3 to 12 months after operation, and the experimental group were significantly lower than those in the control group;compared with the preoperative period, 12 months after operation, the proportion of CD3+, CD4+ in both groups and the ratio of CD3+/CD4+ in the experimental group showed a significant increase trend, and the experimental group was higher than the control group;compared with the preoperative period, nasal resistance of the two groups significantly reduced 12 months
