摘　　要：目的对南京市妇幼保健院母乳库运行现状进行总结和分析,旨在为其他地区母乳库的建立和运行提供借鉴参考。方法对2013年8月—2018年7月南京市妇幼保健院母乳库基本运行情况进行描述性分析。2016年1月起,母乳库进行持续质量改进,通过加大母乳喂养及母乳捐赠宣传力度、加强捐赠母乳质量监管等一系列措施,比较质量改进前后2个阶段母乳库母乳捐赠合格者以及受捐新生儿基本特征。结果我院母乳库自建库至2018年7月,5年来收到捐赠母乳共计4340.84 L,捐赠人数达1401人,总捐赠次数达12368次。母乳库运行2个阶段母乳捐赠合格者文化程度、捐赠次数、分娩孕周、分娩胎次比较,差异有统计学意义(P均<0.001)。运行5年期间,共有5728名新生儿受益,个人接受捐赠母乳量最多为12895 mL,使用捐赠母乳时间最长达53 d。母乳库运行2个阶段受捐新生儿出生体质量、出生孕周、初次使用捐赠母乳时间、使用捐赠母乳持续时间比较,差异有统计学意义(P均<0.001)。结论我院母乳库运行良好,母乳库持续质量改进有效提高了母乳捐赠合格率,母乳捐赠合格人数和受捐新生儿数量明显增加,为新生儿营养支持提供了重要保障。Objective To summarize and analyze the current operation of milk bank in Nanjing Maternity and Child Health Care Hospital,so as to provide references for the establishment and operation of milk banks in other areas.Methods Descriptive analysis was performed to the operation of milk bank in Nanjing Maternity and Child Health Care Hospital from August 2013 to July 2018.Since January 2016,the milk bank has made continuous improvement in quality by taking a series of measures to scale up the promotion of breast feeding and milk donation and to strengthen the supervision on the quality of donated milk.Given this,the basic characteristics of qualified milk donors and newborns accepting donations were compared before and after the quality improvement.Results From July 2018 when the milk bank was established,a total of 4340.84 L of milk has been donated by 1401 donors in 12368 times in the past 5 years.There were significant differences in education level,number of donations,gestational weeks at delivery and parity among the qualified milk donors before and after the quality improvement of milk bank(P<0.001).During 5 years of operation,a total of 5720 newborns were benefited,with individuals receiving up to 12895 mL for a maximum of 53 days.There were significant differences in birth weight,gestational weeks at delivery,the time of first acceptance,and duration of accepting donation among the newborns benefited before and after the quality improvement of milk bank(P<0.001).Conclusion The milk bank in our hospital is running well.By continuous quality improvement,the acceptance rate of donation,and the number of qualified donors and benefited newborns are significantly increased,providing an important guarantee for newborn nutrition support.
