中药薰蒸联合针灸治疗顽固性面瘫的疗效及对面神经功能的影响

摘　　要：目的观察中药薰蒸联合针灸治疗顽固性面瘫的疗效及对面神经功能的影响。方法选择146例顽固性面瘫患者,随机分为对照组和治疗组,每组73例。对照组采用中药薰蒸治疗,治疗组采取中药薰蒸联合针灸治疗,比较两组疗效、面部残疾指数[面部残疾指数(FDI)量表]、面神经功能状况[House-Brackmann(H-B)面部神经功能评分]、面神经电生理(F波潜伏期与时程、瞬目反射R1波潜伏期)状况。结果治疗组疗效优于对照组(P<0.05);两组治疗后FDI量表的社会生活功能(FDIS)评分较治疗前降低(P<0.05),面部残疾躯体功能指数(FDIP)评分较治疗前提高(P<0.05),且治疗组治疗后FDIS评分低于对照组(P<0.05),FDIP评分高于对照组(P<0.05);治疗8周内,两组H-B面部神经功能评分为下降趋势,差异具有统计学意义(P<0.05),且治疗组治疗4周、治疗8周后的H-B面部神经功能评分均低于对照组(P<0.05);两组F波潜伏期与瞬目反射R1波潜伏期缩短(P<0.05),F波时程延长(P<0.05),且治疗组治疗后F波潜伏期与瞬目反射R1波潜伏期短于对照组(P<0.05),F波时程长于对照组(P<0.05)。结论顽固性面瘫患者以中药薰蒸联合针灸方案治疗疗效显著,能改善患者面部残疾与面部神经电生理状况,促进面部神经功能恢复。Objective To observe the therapeutic efficacy of Chinese medicinal fumigation plus acupuncture-moxibustion in treating intractable facial paralysis and its effect on facial nerve function.Method A total of 146 patients with intractable facial paralysis were randomized into a control group and a treatment group,with 73 cases in each group.The control group was intervened by Chinese medicinal fumigation,while the treatment group was given Chinese medicinal fumigation plus acupuncture-moxibustion.The two groups were compared with each other in terms of therapeutic efficacy,Facial Disability Index(FDI,FDI scale),facial nerve function[House-Brackmann(H-B)Facial Nerve Grading],and electrophysiology of the facial nerve(F-wave latency and term and R1-wave latency in blink reflex).Result The treatment group produced more significant efficacy than the control group(P<0.05).After treatment,the FDI-social function(FDIS)score dropped and the FDI-physical function(FDIP)score increased in both groups and the FDIS score was lower in the treatment group than in the control group(P<0.05)and the FDIP score was higher in the treatment group than in the control group(P<0.05).During the 8-week treatment,the H-B facial nerve function score presented a descending tendency in both groups,and the intra-group differences were significant(P<0.05);the H-B facial nerve function score was lower in the treatment group than in the control group at treatment weeks 4 and 8(P<0.05).The F-wave latency and R1-wave latency in blink reflex shortened in both groups(P<0.05),and the F-wave term extended(P<0.05);the F-wave latency and R1-wave latency in blink reflex were shorter in the treatment group than in the control group after intervention(P<0.05),and the F-wave term was longer in the treatment group than in the control group(P<0.05).Conclusion Chinse medicinal fumigation plus acupuncture-moxibustion can produce significant efficacy in treating patients with intractable facial paralysis.It can improve the patients’facial disability and facial nerve
