摘 要:随着我国人口老龄化问题日趋严重,老年性疾病应引起重视。贫血是老年患者最常见的问题之一,不仅提示机体存在潜在疾病,而且与老年患者的体能、认知功能、住院率及病死率的上升有关。老年性贫血的原因复杂多样,主要包括营养缺乏性贫血、慢性病贫血(ACD)以及不能解释的老年性贫血(UA)。营养性贫血主要为铁、叶酸及维生素B 12缺乏导致;ACD主要通过影响肾脏、肝脏以及网状内皮系统而直接或间接影响促红细胞生成素或铁调素的水平,最终导致贫血;UA的机制则比较复杂,主要与克隆性造血有关,另外,UA与ACD可能有共同的生理学基础。With the aggravating of the population ageing in China,the senile disease should be paid more attention.Anemia is one of most common disease in the elderly population,which not only indicates a potential disease in the body,but is also associated with physical fitness,cognitive function disorder,high hospitalization rate and the fatality rise in the elderly patients.The causes of senile anemia are complex and diverse,mainly including:nutritional deficiency anemia,anemia of chronic disease(ACD)and unexplained anemia(UA)in the elderly.Nutritional deficiency anemia is mainly caused by iron,folic acid and vitamin B 12 deficiency;in ACD the level of erythropoietin or hepcidin is affected directly or indirectly by affecting the kidney,liver and reticuloendothelial system,eventually leading to anemia;the mechanism of UA in the elderly is more complicated and is mainly related to clonal hematopoiesis.Studies have also shown that UA may have a common physiological basis with ACD.
