广西贵港市50岁以上暗娼人群艾滋病哨点监测结果及HIV感染因素分析

摘　　要：目的了解贵港市50岁以上暗娼人群高危行为,HIV/梅毒感染及影响因素,为制定切实有效的防治对策提供科学的理论依据。方法按照《全国艾滋病哨点监测实施方案》要求抽取调查对象,由经过培训的调查人员使用"国家艾滋病哨点监测FSW调查问卷"进行面对面匿名咨询调查。调查其艾滋病相关知识及行为情况等,同时采血检测艾滋病、梅毒感染情况。数据分析采用SPSS 19.0软件。结果共调查306名50岁以上暗娼,平均年龄(52.92±3.04)岁,文化程度以小学为主,占63.40%;在婚或同居91.18%,艾滋病相关知识知晓率为96.41%。首次发生商业性行为的平均年龄为(37.98±8.95)岁;最近一个月内接客数为(70.89±63.46)次,平均收费价格为(39.95±20.92)元,与商业性伴发生异性性行为时坚持使用安全套的占75.49%。HIV、梅毒感染率分别为8.82%、24.84%。HIV多因素分析仅发现,离异或丧偶是50岁以上暗娼人群HIV感染的危险因素(OR=4.03,95%CI:1.41~11.50)。结论贵港市艾滋病通过50岁以上暗娼人群传播的风险很高,大龄暗娼更需要重点关注;同时,要有针对性地提高艾滋病、性病有关知识的宣传教育和高危行为的干预有效性,增强其自我防护和安全套使用意识,降低感染艾滋病、性病的风险。Objective To explore the high-risk behaviors,HIV/syphilis infection and their influencing factors among female sex workers(FSWs)over 50 years old in Guigang City,to provide a scientific basis for control measures.Methods According to the National AIDS Sentinel Surveillance Standards,a face-to-face anonymous consultation was carried out by trained investigators using National AIDS Sentinel Surveillance of FSWs questionnaire in order to understand the HIV related knowledge and behaviors of FSWs.And their blood samples were tested for HIV and syphilis infection.The data were analyzed by SPSS 19.0.Results A total of 306 FSWs over 50 years old were investigated,with an average age of(52.92±3.04).And they mainly had primary school education,which accounted for 63.40%.91.18%of them were married or cohabiting.The awareness rate of HIV related knowledge was 96.41%.The average age of first commercially sexual behavior was(37.98±8.95).The average number of commercially sexual behavior in the last month was(70.89±63.46)and the average charge price was(39.95±20.92)yuan.In commercially sex sexual behavior,75.49%of workers can insist on using condoms.The infection rate of HIV and syphilis were 8.82%and 24.84%,respectively.HIV multivariate analysis found that divorce and bereft of one’s spouse were risk factors for HIV infection(OR=4.03,95%CI:1.41~11.50).Conclusion Guigang had a high risk of HIV transmission through sex workers over 50 years old,so more attention should be paid to the older FSWs.At the same time,it is necessary to improve the effectiveness of publicity and education about HIV or sexually transmitted diseases(STDs),as well as the intervention of high-risk behaviors,in order to enhance their awareness of self-protection and condom use and reduce the risk of HIV and STDs infection.
