摘 要:目的研究深圳市宝安区乙肝疫苗和免疫球蛋白阻断乙肝母婴传播效果。方法调查2016年全区所有接种门诊中HBsAg阳性产妇所生的、出生24 h内已接种乙肝免疫球蛋白、且出生后已完成3针乙肝疫苗接种的6~24月龄婴幼儿共879名,随访调查血清中HBsAg和HBsAb阳性率及分娩和喂养方式等。结果HBsAg阳性率0.34%,HBsAb阳性率96.70%,不同年龄组HBsAb阳性检出率差异有统计学意义(χ^2=19.3685,P<0.01)。母乳喂养和人工喂养婴幼儿的HBsAb阳性率差异有统计学意义(χ^2=9.0186,P<0.01)。未发现喂养方式、分娩方式、是否早产、性别、监测月龄与阻断效果有关。结论乙肝疫苗联合免疫球蛋白阻断乙肝母婴传播效果好。Objective To observe the blocking effects of hepatitis B vaccine and immune globulin(HBIG)on the maternal-infant hepatitis B virus transmission.Methods A total of 879 infants aged 6~24 months,born in 2016 in Bao’an District and of mothers with positive hepatitis B surface antigen(HBsAg)were monitored.And all of them had been inoculated with HBIG at birth,and hepatitis B vaccine(HBV)for three times.The positive rates of HBsAg and HBsAb,the feeding method and delivery mode were surveyed.Results The positive rates of HBsAg and HBsAb in infants were 0.34%and 96.70%,respectively.A statistically significant difference was found in the positive rate of HBsAb among different age groups(χ^2=19.3685,P<0.01).A statistically significant difference was found in the positive rate of HBsAb between breast-fed and bottler-fed infants(χ^2=9.0186,P<0.01).No correlation was found between the blocking effect and feeding methods,delivery mode,gender,age monitoring or premature birth.Conclusion HBIG had significant effects on the interruption of mother-to-infant hepatitis B virus transmission.
