摘　　要：目的分析安康市2018年疑似预防接种异常反应(adverse event following immunization,AEFI)的发生特征,评价AEFI监测系统运转情况,调查核实AEFI发生情况和原因,为提高预防接种服务质量提供依据。方法通过中国免疫规划信息系统收集2018年AEFI个案数据,采用描述性方法对相关指标进行分析。结果安康市2018年报告AEFI 323例,其中一般反应317例、异常发应5例、偶合症1例,报告发病率43.40/10万剂次。男、女性别比1.5∶1,≤1岁组儿童260例,占80.50%。发生分类以一般反应为主,症状以发热、红肿、硬结为主。报告发生率居前3位的疫苗分别为:百白破联合疫苗、麻风疫苗和甲肝疫苗。48 h内报告率99.07%,48 h内调查率100.00%,AEFI分类率100.00%。结论安康市AEFI监测工作敏感性较高,达到全国监测方案标准,所使用的的疫苗安全,但仍需继续做好监测、报告和处置工作。Objective To analyze the features of the adverse effect following immunization(AEFI)in Ankang City in 2018,evaluate the operation of AEFI surveillance system and investigate the reasons for AEFI,in order to provide some evidences to improve the quality of human vaccination.Methods The data of AEFI cases were collected from the National Immunization Information System,then descriptive methodology was used to analyze the related indicators.Results Totally 323 AEFI cases were reported in 2018,including 317 cases of general reactions(98.14%),5 cases of paradoxical reactions(1.55%)and 1 case of coincidental symptom(0.31%).The reported incidence was 43.40/100000,and the male to female ratio was 1.50∶1.Totally 260 cases were children under one year old,accounting for 80.50%.General reactions was most seen in cases,and the mainly symptom included fever,local swelling and indurations.The top three responsible vaccines of reported incidence were DTap,MR and HepA.The reported rate in 48 hours was 99.07%,the investigated rate in 48 hours was 100.00%,and AEFI classification rate was 100.00%.Conclusion The surveillance of AEFI in Ankang City was sensitive,which has reached the national standard of surveillance program,and the vaccines used were safe.However,surveillance,reporting and disposal work should be continued.
