摘  要:目的对一起学校结核病聚集性疫情发生的原因进行调查分析,为今后学校结核病疫情防控制定科学对策。方法通过追踪核查首发病例的诊治过程,密切接触者进行PPD(purified protein derivative of tuberculin)实验筛查和胸部X线检查,同时开展患病学生的流行病学调查,以了解此次疫情情况。结果高二高三年级1644名师生接受了PPD试验,其中强阳性109人,确诊活动性肺结核病人24例,其中首发病例所在班级发病最多,为16例,占发病总数的66.67%。此次疫情中师生肺结核罹患率为0.70%,疫情传播范围主要为与首发病例密切接触。结论疾控机构及学校在《结核病防治工作指南》的指导下对患病学生主动追踪调查,及时发现和阻断肺结核的传染源及传播途径,可有效控制学生结核疫情的发生发展。Objective To investigate the cause of one case of tuberculosis cluster,and to provide a reliable reference for the control and prevention of tuberculosis in school.Methods Investigation of diagnosis and treatment process of the first case,(purified protein derivative of tuberculin)PPD test and chest X-ray of the close contacts,and the epidemiological survey of infected students were carried out to analyze this epidemic.Results Totally 1644 students and teachers of junior and senior class received PPD test.And a total of 109 people were tested strongly positive,and 24 were diagnosed as active tuberculosis.The incidence was mainly concentrated in the class where the first infected student was found,with 16 case(66.67%)diagnosed as tuberculosis.The prevalence rate in this epidemic was 0.70%,and the main transmission route was close contact with the first infected case.Conclusion Under the Guidelines for Tuberculosis Prevention and Control,disease control institutions and schools can effectively control the development of tuberculosis among students by actively tracking and timely interrupting the transmission of tuberculosis.
