摘　　要：目的比较电视辅助胸腔镜(VATS)肺叶切除术与开胸肺叶切除术治疗慢性阻塞性肺疾病(COPD)合并非小细胞肺癌(NSCLC)术后并发症发生情况。方法选取自2010年1月至2016年12月辽宁省肿瘤医院收治的182例COPD合并NSCLC患者为研究对象。按手术方式不同将患者分为A组与B组,每组各91例。A组采用开胸肺叶切除术治疗;B组采用VATS肺叶切除术治疗。比较两组患者肺炎发生率、肺并发症发生率、手术时间及住院时间。结果B组手术时间、住院时间短于A组,两组比较,差异有统计学意义(P<0.05)。B组肺并发症发生率为1.1%(1/91),显著低于A组的12.1%(11/91),两组比较,差异有统计学意义(P<0.05)。B组术后肺炎发生率为1.1%(1/91),显著低于A组的11.0%(10/91),两组比较,差异有统计学意义(P<0.05)。结论与开胸肺叶切除术相比,VATS肺叶切除术治疗COPD合并NSCLC患者的术后肺并发症较少,且住院时间较短,具有一定的优势。Objective To compare the postoperative complications of chronic obstructive pulmonary disease(COPD)complicated with non-small cell lung cancer(NSCLC)after video-assisted thoracoscopic surgery(VATS)or thoracotomy lobectomy.Methods A retrospective study was performed on 182 cases of patients with COPD complicated with NSCLC who were admitted from January 2010 to December 2016.Patients were divided into Group A and Group B according to different surgical methods,with 91 cases in each group.Group A of patients received thoracotomy.Group B of patients received VATS.The incidence of pneumonia,incidence of pulmonary complications,operation time and length of stay were compared between the two groups.Results The operation time and hospitalization of Group B were shorter than that of Group A,and the difference between the two groups was statistically significant(P<0.05).The incidence of pulmonary complications in Group B was 1.1%(1/91),which was significantly lower than that in Group A(12.1%),P<0.05.The incidence of postoperative pneumonia in Group B was 1.1%(1/91),which was significantly lower than that in Group A(11.0%,10/91),P<0.05.Conclusion Compared with thoracotomy,VATS for COPD patients with NSCLC has certain advantages as it has fewer postoperative pulmonary complications and shorter hospitalization time.
