摘 要:目的:通过低剂量控制技术,即ASiR-V全模型自适应统计迭代技术、低千伏、短扫描时间、低辐射剂量在新生儿缺血缺氧性脑病HIE诊断中的临床应用,对照常规扫描方式,在降低患儿辐射剂量的前提下,HIE的检出率是否会有变化。方法:选取2018年3月-2019年3月南京医科大学附属常州妇幼保健院收治的新生儿HIE的患者80例,随机分为低剂量控制技术检查组与正常检查组各40例,低剂量控制技术组采用管电压70kV,采用ASiR-V全模型迭代技术调制管电流(20~400mA),准直宽度0.625mm,机架转速0.28S/r,螺距0.5;正常检查组采用管电压110kV,传统滤波反投影法重建技术,自动调制管电流(20~400mA),准直宽度0.625mm,机架转速0.28S/r,螺距0.5,对两组的图像质量、HIE检出率、图像信噪比(SNR)、辐射剂量值进行评价。结果:低剂量控制技术组与正常组(P<0.05):两组图像质量、HIE检出率,差异无统计学意义(P>0.05);图象信噪比(SNR),差异有统计学意义(P<0.01);辐射剂量值:CT剂量指数CTDI、计量长度乘积DLP、有效剂量ED,低剂量控制技术技术组为(10.07±1.33)mGy、(31.45±2.45)mGy*cm、(0.50±0.04)mSV,正常检查组为(20.12±1.87)mGy、(107.31±5.23)mGy*cm、(1.72±0.09)mSV,CTDI减少50%,DLP减少70%,低剂量控制技术组明显低于正常检查组(P<0.01),差异有统计学意义。结论:低剂量控制技术在新生儿HIE检查中,所获得的图像质量不会影响HIE的诊断,而低剂量控制技术明显降低新生儿的检查辐射剂量。Objective To evaluate the clinical application of low dose control techniques,namely ASiR-V full model adaptive statistical iteration technique,low kilovolt,short scan time and low radiation dose,in the diagnosis of neonatal hypoxic-ischemic encephalopathy(HIE),and to compare with routine scanning methods,whether the detection rate of HIE will change under the premise of reducing radiation dose.Methods 80 cases of neonatal HIE admitted to Changzhou Maternal and Child Health Hospital Affiliated to Nanjing Medical University from March 2018 to March 2019 were randomly divided into two groups:low-dose control technology group and normal control group,40 cases in each group.The low-dose control technology group adopted tube voltage 70kV and ASiR-V full-model iteration technology to modulate tube current(20~400mA),collimation width 0.625mm,rack speed 0.28S/r and pitch 0.5mA.The normal inspection group used 110kV tube voltage,traditional filter back projection reconstruction technology,automatic modulation tube current(20~400mA),collimation width of 0.625 mm,rack speed of 0.28S/r,pitch of 0.5.The image quality,HIE detection rate,image signal-to-noise ratio(SNR),radiation dose of the two groups were evaluated.Results There was no significant difference in image quality and HIE detection rate between low dose control group and normal group(P<0.05).There was significant difference in image signal-to-noise ratio(SNR)between the two groups(P<0.01).Radiation dose value:CT dose index CTDI,measurement length product DLP,effective dose ED,low dose control technology group was(10.07±1.33)mGy,(31.45±2.45)mGy*cm,(0.50±0.04)mSV,normal examination group was(20.12±1.87)mGy,(107.31±5.23)mGy*cm,(1.72±0.09)mSV,CTDI decreased by 50%,DLP decreased by 70%,low dose control technology group was significantly lower than normal examination group(P<0.01),the difference was statistically significant.Conclusions The image quality obtained by low dose control technology in neonatal HIE examination will not affect the diagnosis of HIE,while l
