摘　　要：[目的]探讨社区2型糖尿病病人的心理痛苦现状及影响因素。[方法]采用糖尿病痛苦量表(DDS)与2型糖尿病病耻感评估量表(DSAS‑2)对蚌埠市2个社区279例糖尿病病人进行问卷调查。[结果]52.0%社区2型糖尿病病人存在糖尿病痛苦,DDS得分为(2.15±0.82)分,得分最高的维度是生活规律相关痛苦[(2.44±1.04)分],最低的是人际关系相关痛苦[(1.85±1.02)分]。不同文化程度、是否有并发症、是否使用胰岛素病人DDS得分比较差异有统计学意义(P<0.05)。社区2型糖尿病病人DSAS‑2总分及其各维度得分均与DDS总分及其各维度得分呈正相关(P<0.01)。多元逐步回归分析显示,文化程度和病耻感是社区2型糖尿病病人糖尿病痛苦的影响因素(P<0.05)。[结论]社区工作人员应密切关注糖尿病病人的心理痛苦和病耻感,加强对糖尿病病人的心理干预,提高病人的生活质量。Objective:To explore status quo of diabetes distress of patients with type 2 diabetes in communities and its influencing factors.Methods:A questionaire survey,involving 279 patients with type 2 diabetes in 2 communities in Bengbu city,was conducted by using Diabetes Distress Scale(DDS)and Type 2 Diabetes Stigma Assessment Scale(DSAS‑2).Results:Patients with diabetes distress accounted for 52.0%of patients with type 2 diabetes in communities.Score of DDS of patients with diabetes distress was 2.15±0.82.The dimension with the highest score was the regimen‑related distress,with a score of 2.44±1.04.The dimension with the lowest score was the distress related to interpersonal relationship,with a score of 1.85±1.02.There were statistically significant differences in DDS scores of patients with different education level,with or without complications and insulin therapy(P<0.05).The total score of DSAS‑2 and score of each dimension were positively correlated with the total score of DDS and score of each dimension(P<0.01).Multiple linear regression analysis showed that education level and stigma were the influencing factors of diabetes distress among patients with type 2 diabetes in communities(P<0.05).Conclusion:Working staff in communities should pay close attention to the diabetes distress and stigma of diabetic patients,strengthen psychological interventions of diabetic patients,and improve their quality of life.
