内异消癥汤对轻度子宫内膜异位症患者子宫动脉血流动力学的影响

摘　　要：目的运用经阴道彩色多普勒超声技术评估内异消癥汤对子宫内膜异位症(EMs)合并不孕症患者子宫动脉血流的影响。方法将60例轻度EMs患者分为治疗组与对照组,治疗组于手术治疗前服用内异消癥汤1个疗程(3个月经周期),对照组在等待择期手术前不进行中医治疗。观察2组术前最后1次排卵前子宫动脉血流动力学参数:收缩期最大流速(PSV),舒张末期流速(EDV),搏动指数(PI),阻力指数(RI),并记录术后1年内的妊娠率和妊娠结局。结果治疗组PSV显著高于对照组(P<0.05),PI、RI均显著低于对照组(P<0.05),EDV略高于对照组,但无统计学差异(P>0.05);术后1年内妊娠率,治疗组显著高于对照组(P<0.05),妊娠结局优于对照组(P<0.05)。结论内异消癥汤可有效改善轻度EMs患者子宫动脉血流动力学状态,提高手术治疗后妊娠率,改善妊娠结局。Objective:To evaluate the effect of Neiyixiaozheng Decoction on the treatment of uterine artery hemodynamics in patients with mild endometriosis(EMs)with infertility by transvaginal color doppler ultrasound technology.Methods:60 patients with mild EMs were divided into a treatment group and a control group.The treatment group took Neiyixiaozheng Decoction for 1 course of treatment(3 menstrual cycles)before surgery,and the control group did not undergo TCM treatment before waiting for elective surgery.The hemodynamic parameters of the uterine artery before ovulation,peak systolic flow veiocity(PSV),end diastolic flow veiocity(EDV),pulsatility index(PI),resistance index(RI)in the 2 groups before operation were observed,and the pregnancy rate and pregnancy outcome during the year were recorded after operation.Results:The PSV of the treatment group was significantly higher than that of the control group(P<0.05),The PI and RI were significantly lower than those of the control group(P<0.05),and the EDV was slightly higher than that of the control group,but there was no statistical difference(P>0.05).The pregnancy rate within the next year was significantly higher in the treatment group than in the control group(P<0.05),and the pregnancy outcome was better than that in the control group(P<0.05).Conclusion:Neiyixiaozheng Decoction can effectively improve the uterine artery hemodynamics in patients with mild Ems,increase the pregnancy rate after surgery and improve the pregnancy outcome.
