摘　　要：髌骨下极是指髌骨远端1/4无关节软骨覆盖的部分,此部分骨折是一类特殊的非关节面骨折,AO/OTA分型为34A1。髌骨远端有髌韧带附着,传递股四头肌肌力参与伸膝活动的完成,是构成伸膝装置的重要组成部分。手术治疗的目的之一是恢复伸膝装置的完整性,重建髌韧带。近年来随着骨折治疗理念的革新以及新型内固定物的出现,临床医师提出了多种手术策略,除广泛应用的张力带固定技术外,还包括髌骨周围环扎、聚髌器、特殊钢板固定、髌骨部分切除、带线锚钉重建髌韧带等,但各有利弊,尚未有权威的治疗选择。The inferior pole of the patella refers to the part of the distal patella which is not covered by articular cartilage,and fractures of this position is a special kind of extra-articular fracture.According to the AO/OTA classification,it is 34A1.The distal patella has patellar ligament attachment,which transfers quadriceps femoris muscle strength and participates in the completion of knee extension activities,constituting an important part of the knee extension device.The goal of surgery is to restore the integrity of the knee extension device and reconstruct the patellar ligament.In recent years,with the innovation of fracture treatment concept and the emergence of new internal fixators,clinicians have put forward a variety of surgical strategies.In addition to the widely used tension band technology,other treatment methods include peripatellar circumferential ligation,patellar concentrator,special plate fixation,partial patellectomy,patellar tendon reconstruction with anchor suture,although each has its own advantages and disadvantages,and has not yet formed an authoritative choice of treatment.
